肺移植术后患者床边胸片图像质量因素分析

摘　　要：目的探讨影响肺移植术后患者床边胸片图像质量的因素,并提出改良措施。方法对南京医科大学附属无锡市人民医院65位肺移植术后患者床边胸片528例的图像资料,提出质量评估标准,将其分为甲级片、乙级片、废片三级,并进行系统性分析。结果肺移植术后患者床边胸片528例中:甲级片243例(46.0%),乙级片274例(51.9%),废片11例(2.1%)。乙级片主要原因为投照体位欠佳和胸部伪影,少数为呼吸运动和曝光参数过量或不足;废片多数为由于曝光参数过量或不足所致,少数为呼吸运动和胸部伪影太多。结论床边摄影本身的局限性以及肺移植术后患者病情危重、监护设施的影响,使得床边胸片图像质量较差,废片率较高,可以通过相应改良措施来提高图像质量。[Objective]To study the factors influencing image quality of bedside chest radiography in patients after lung transplantation,put forward the improvement measures. [Methods]528 image materials of bedside chest radiography of 65 patients after lung transplantation in Wuxi People＇s Hospital Affiliated to Nanjing Medical University were collected.According to the quality assessment criteria,the images were classified into grade A film,grade B film and waste film,and were analyzed comprehensively. [Results]Among 528 images,there were 243（46.0%） films of grade A,274（51.9%） films of grade B and 11（2.1%） waste films.The main reasons of grade B films were poor posture and artifact,followed by respiratory movement,excessive or inadequate exposure parameters.Most of waste films were caused by excessive or inadequate exposure parameters,followed by respiratory movement and artifact. [Conclusion]Because of limitation of bedside radiography,critical condition of patients after lung transplantation and impact of monitoring facilities,the image quality of bedside chest radiography is poor,and the waste film rate is high.The appropriate measures can improve the image quality.
