摘 要:目的比较植入多焦点人工晶状体(MIOL)与单焦点人工状晶体(SIOL)患者术后视力和阅读视力的差异,评价多焦点人工晶状体植入的有效性及优越性。方法将74例(120只眼)老年性白内障患者按自愿选择的原则分为两组,每组60眼,行超声乳化白内障吸出术,MIOL组植入Alcon Acrysof ReSTOR多焦点人工晶状体,SIOL组植入Alcon Acrysof Natural单焦点人工状晶体。术后3个月对裸眼远视力、最佳矫正远视力、裸眼近视力、最佳矫正近视力,最佳矫正远视力下的近视力、63cm和100cm中视力、裸眼阅读视力、矫正近视力下阅读视力和矫正远视力下阅读视力等视功能进行检查。结果两组裸眼远视力、矫正远视力及矫正近视力无明显差异(P〉0.05);裸眼近视力、矫正远视力下近视力、63cm和100cm中视力,MIOL组明显优于SIOL组(P〈0.05);植入MIOL眼裸眼阅读视力和矫正远视力下阅读视力均优于植入SIOL眼,裸眼近视力与裸眼阅读视力,矫正远视力下近视力与矫正远视力下阅读视力间分别存在直线相关关系(r1=0.802,r2=0.816);直线回归方程为:Y1=3.995+13.522X,(P〈0.05),Y2=3.696+13.122X2(P〈0.05)(x1=裸眼近视力,YI=裸眼阅读视力;x2=矫正远视力下近视力,Y2=矫正远视力下阅读视力)。结论MIOL植入有效而优越,可以提供与SIOL一样良好的远视力,但近视力、中距离视力及阅读视力更好。良好的近视力是阅读视力的基础。Objective To determine and compare the reading visual acuity (RVA) and the visual outcomes after implantation of multifocal intraocular lenses (MIOL) and monofocal intraocular lenses (SIOL). Methods In this study, 74 patients with age-related cataract (120 eyes) received phacoemulsification and MIOUSIOL implantation respectively according to their own will. Sixty eyes were implanted with Alcon AcrySof ReSTOR SA60D3 (MIOL group), the other 60 eyes were implanted with Alcon AcrySof Natural SN60AT (SIOL group). Uncorrected distance visual acuity (UCDVA), best corrected distance visual acuity (BCDVA), uncorrected near visual acuity (UCNVA), best corrected near visual acuity (BCNVA), distant corrected near visual acuity (DCNVA), 63cm and 100cm intermediate visual acuity (IMVA), uncorrected reading visual acuity (UCRVA), near corrected reading visual acuity (NCRVA) and distant corrected reading visual acuity (DCRVA) were observed 3 months postoperatively. Results There was no statistically significant difference between the 2 groups at UCDVA, BCDVA and BCNVA (P 〉0.05). MIOL eyes showed better UCNVA, DCNVA, IMVA, UCRVA and DCRVA than the SIOL eyes (P 〈0.05). There were linear correlations between UCNVA and UCRVA (rt=0. 802 ), DCNVA and DCRVA (r2=0. 816 ). Linear regression equation respectively : Y1=3.995+13.522X1 (P 〈0.05), Y2=3.696+13.122X2 (P 〈0.05) (X1= UCNVA, Y1=UCRVA; X2=DCN-VA, Y2=DCRVA). Conclusions MIOL is effective and superiority. It can provide good distance visual acuity (DVA) as the SIOL does. Meanwhile, it can provide better UCNVA, DCNVA, IMVA, UCRVA and DCRVA. Good near visual acuity (NVA) is the basement of good RVA.
