摘  要:目的探讨高级别胶质瘤中基质金属蛋白酶(matrix metalloproteinase,MMP)-2活性及其中介体与患者预后的关系。方法 26例高级别胶质瘤患者,进行明胶酶谱定量检测,将MMP-2及其中介体的表达情况与患者术后生存时间进行统计学分析。结果Ⅲ级和Ⅳ级胶质瘤MMP-2水平差异有统计学意义(P=0.000),MMP-2水平与分级呈正相关(rs=0.727,P=0.000);Ⅲ级和Ⅳ级胶质瘤中均有MMP-2中介体出现,在Ⅳ级胶质瘤中出现中介体的患者生存时间少于未出现中介体的患者(P=0.002);Cox回归分析显示,高级别胶质瘤中MMP-2的水平及其中介体的有无是影响患者生存时间的独立预后因子(P=0.003,P=0.004)。结论高级别胶质瘤中MMP-2水平与其中介体的存在与否是影响患者术后生存时间的重要因素。Objective To study the correlation of matrix metalloproteinase-2 and its intermediate form in high-grade glioma with the prognosis.Methods Twenty-six cases of high-grade glioma tissue were examined with gelatin zymography.The expressions of matrix metalloproteinase-2 and its intermediate form as well as the survival time of patient were analyzed.Results There was a significant difference in the level of matrix metalloproteinase-2 between glioma tissues of grade Ⅲ and Ⅳ(P=0.000).The level was positively correlated with the grading(rs=0.727,P=0.000).Intermediate matrix metalloproteinase-2 was detected both in glioma tissues of grade Ⅲ and Ⅳ.The survival time was shorter in patients with intermediate matrix metalloproteinase-2 than that in the patients without intermediate matrix metalloproteinase-2(P=0.002).Cox regression analysis showed that level of active matrix metalloproteinase-2 and existence of intermediate form were the independent prognostic factors for the survival time(P=0.003,P=0.004).Conclusion The level of matrix metalloproteinase-2 in high-grade glioma and the existence of intermediate form are important prognostic factors for the survival time.
