两阶段法远端胃切除+D2淋巴结清扫在胃窦癌根治术中的临床研究

摘　　要：目的探讨两阶段法远端胃切除+D2淋巴结清扫在胃窦癌根治术中的临床疗效。方法回顾性分析76例因胃窦癌行远端胃切除+D2淋巴结清扫的胃癌患者的临床病理资料,根据其手术方案不同随机分为两阶段法手术组和常规手术组。统计2组手术时间、淋巴结清扫个数、手术出血量、住院时间、术后并发症和5年生存率。结果两阶段法手术组较常规手术组手术时间、手术出血量明显减少,但淋巴结清扫个数、住院时间、术后并发症和5年生存率和与常规手术组比较,差异无统计学意义。结论应用两阶段手术进行远端胃切除+D2淋巴结清扫可显著缩短胃窦癌根治术的手术时间,减少出血量,并保证手术根治性与疗效。Objective To explore the clinical effect of 2 stages distal gastrectomy and D2 lymph nodes radical dissection on gastric antrum cancer.Methods Clinical data on 78 patients of 2 stages distal gastrectomy and D2 lymph nodes radical dissection on gastric antrum cancer were retrospectively analyzed.The patients were randomly divided into two groups： 2 stages group and traditional group.The comparisons of the mean operative time,the blood loss,number of lymph nodes dissection,mean time of hospital stay,complication and 5 years survival rate were done.Results There were statistically significant differences of the mean operative time and the blood loss,and there were no differences of lymph nodes dissection,complications and the mean time of hospital stay between the two groups.Conclusion Two stages D2 radical dissection with distal subtotal gastrectomy for distal gastric cancer can shorten the operative time,reduce the blood loss,And this method is helpful to improve the radical treatment of dissection.
