异甘草酸镁在结直肠癌FOLFOX方案辅助化疗中护肝作用观察

摘　　要：目的研究异甘草酸镁在结直肠癌术后FOLFOX方案辅助化疗中,对药物性肝损害的防治作用。方法选择本院收治的结直肠癌术后辅助化疗采用FOLFOX方案的患者87例,随机分为A、B、C 3组,A组从每周期化疗首日开始预防性使用异甘草酸镁注射液,在出现肝损害后,使用异甘草酸镁注射液合并葡醛内酯注射液进行治疗;B、C组不预防应用护肝药物,在化疗开始后观察到肝损害时,B组予异甘草酸镁合并葡醛内酯注射液进行治疗,C组予甘草酸二胺注射液合并葡醛内酯注射液进行治疗。治疗开始首日起观察:血清丙氨酸氨基转移酶(ALT)、血清天门冬酸氨基转移酶(AST)、血钾、血压和浮肿的变化。结果 A组高分级肝损害发生率略低于B组,差异无统计学意义,但明显低于C组,差异有统计学意义。3组不良反应发生率差异无统计学意义。结论异甘草酸镁对结直肠癌患者FOLFOX方案辅助化疗所致药物性肝损害有显著保护作用。Objective To evaluate the efficacy of Magnesium Isoglycyrrhizinate（MI） on preventing drug-induced liver injury（DILI） induced by adjuvant chemotherapy of FOLFOX regimen on colorectal cancer.Methods Eighty-seven patients after resection of colorectal cancer were randomized into three groups.Patients in group A,as preventive measure,received MI during chemotherapy until DILI was observed,and then received MI combined with Glucuronolactone.Patients in group B received MI combined with Glucuronolactone while DILI was observed.Compared with group B,the patients in group C received Diammonium Glycyrrhizinate instead of MI.The levels of ALT,AST,Serum Potassium,blood pressure and edema were evaluated in all patients.Results The high degree of DILI in group A was slightly lower than that in group B,and there was no statistical difference.But it was significantly lower than that in group C,and there was statistical difference between the two groups.There was no statistical difference in rates of adverse reaction among the three groups.Conclusion MI can distinctly protect the liver function of the patients with colorectal cancer from adjuvant chemotherapy of FOLFOX regimen.
