摘 要:目的:探讨宫颈偏移对中央挡铅野及盆腔两侧剂量分布和均匀性的影响,以指导后装放疗(ICRT)时,安装施源器的正确方向。方法:随机测量285例放射治疗后程,SIEMENS-PRIMUS外照射(ExRT)后宫颈癌患者的宫颈管口位置,用无菌钡剂小棉球放入宫颈管口内,测后取出。在SIEMENS-SIMVIEW3000型模拟定位机下测量宫颈口与人体中线(左右间)、前后中平面(前后野中间)相互距离的关系。结果:宫颈口在正中线位的有10例、仅占4.2%(12/285);偏右的有62例,占21.7%(62/285);偏左的有223例,占78.3%(223/285)。其中偏移在2cm以上90例,占31.6%(90/285)。左右最大的偏移为6cm。宫颈口在正中平面位者(前后野中间)有8例.占2.8%(8/285);偏前者有209例,占73.3%209/285);偏后者有76例,占26.7%(76/285)。偏移在2cm以上者有49例,占17.2%(49/285)。前后最大偏移为5em。结论:宫颈口前后移位对放射剂量分布、均匀性无影响;而左右偏移,对挡铅后,算量的放射剂量分布与均匀性有一定影响。Objective:To explore dose distributed and homogeneous effects in central Pb shield field and pelvic field on cervical excursion.Methods:Measure randomly 285 eases cervical tube position of Uterus cervical carcinoma in radiotherapeutic late stage.disinfectant barium small cotton ball be put into cervical tube,take it away after measuring.Under the simulator,measure distance interrelations between uterus cervix and the body middle line (between right and left),and the body front-back middle plane (between front field and back filed).Results:Uterus cervices in body middle line have 10 eaSes,only about 4.2%(12/285); lean to right be aboul 62 cases,only 21.7%(62/285);lean to left be about 223 cases, only 78.3%(223/285).Remove more than 2cm have about 90 cases, only 31.6%(90/285).Maxi-excursion in the right and the left is 6cm.Uterus cervices in the body middle plane (between front field and back field) have 8 cases,only 2.8%(8/285),lean to front be 209 cases,only 73.3%(209/285);lean to back be 76 cases.only 26.7%(76/285).Remove more than 2cm have about 49 cases,only 17.2% (49/285).Maxi-excursion between the front and back is 5cm.Conclusion:Uterus cervix excursion between the front and the bake has no influence on radiologic dose distribution and homogenous.Remove between the left and the right have some influence on radiologic dose distribution and homogenous in central Pb shield field.
