腮腺粘液表皮样癌术后行组织内^(125)Ⅰ放射性粒子治疗临床观察

摘　　要：目的:探讨^(125)Ⅰ放射性粒子在腮腺粘液表皮样癌术后行组织间植入的治疗效果。方法:腮腺粘液表皮样癌病例36例随机分为两组,A组16例腮腺粘液表皮样癌作常规腮腺恶性肿瘤根治手术。B组20例行保留面神经腮腺并肿瘤切除术,尽量将肿瘤切除干净,然后在怀疑有肿瘤残留或安全边界不足处放置125I放射性粒子作为术后辅助治疗。两组病例在性别、年龄、UICC分期、病理分级之间均无显著性差异。术后随访,观察两组病例治疗效果。结果:A组13例有面瘫症状,4例复发;B组无一例出现面瘫症状,无一例复发。结论:腮腺粘液表皮样癌术后辅以放射性粒子植入组织内放疗疗效可靠且方法安全、操作简便,可有效降低腮腺粘液表皮样癌术后并发症和复发率。Objective：To explore the therapeutic effects of postoperatively interstitial radiotherapy of ^（125）Ⅰ radioactive seed implantation for mucoepidermoid carcinoma of parotid gland.Method：The patients were divided randomly into two groups：Group A with 16 cases,were performed with conventional radical resection for malignant parotid gland tumors.Group B with 21 cases,were underwent by parotidectomy and tumor resection with retaining facial nerves,having tumor excision as complete as possible,with ^（125）Ⅰ radioactive seed implantation in doubtedly-tumor-residual or unsafe-bordered places as postoperative adjuvant therapy.And there＇re no obvious differences between the two groups of cases in sex,age,Stage UICC,pathological grades.The therapeutic effects of the two groups were analyzed and observed retrospectively.Result：13 cases of Group A had facial paralysis symptoms,4 of which recurred.None of Group B appeared facial paralysis symptoms or recurred.Conclusion：The therapeutic effects of postoperatively adjuvant interstitial radiotherapy of ^（125）Ⅰ radioactive seed implantation for mucoepidermoid carcinoma of parotid gland are reliable.And the method is safe,simply operative,efficient in reducing the postoperative complications and recurrence rate of mucoepidermoid carcinoma of parotid gland.
