前列腺未分化肉瘤伴软骨化生1例报道并文献复习

摘　　要：目的:探讨前列腺未分化肉瘤伴软骨化生的临床表现和病理特点,进一步明确其所属类别。方法:分析2009年2月收治的1例前列腺未分化肉瘤伴软骨化生患者的临床资料,对该例手术切除的肿瘤组织行常规HE及免疫组化染色,光镜下观察其组织结构和免疫组化表达,并结合文献复习其临床表现。结果:病理诊断:灰白色前列腺肿物,镜下见其含有软骨肉瘤和未分化肉瘤成分。免疫组化染色:波形蛋白(+),局部CD117(±)。平滑肌肌动蛋白、结蛋白、肌红蛋白、CD34、细胞角蛋白CK7、CK8均为(-)。术后2个月发现肿瘤转移,4个月后患者死亡。结论:本例患者为前列腺未分化肉瘤伴软骨化生,该例肿瘤是一种恶性程度较高的侵袭性肿瘤,非常罕见,需经病理检查和免疫组化来确诊。Objective： To investigate the clinical presentations and pathologic features of undifferentiated sarcoma of the prostate with cartilage metaplasia,and to clarify its category.Methods： We analyzed the clinical data of a case of undifferentiated sarcoma of the prostate with cartilage metaplasia treated by surgical resection.The tumor tissue was subjected to routine HE and immunohistochemical staining,its histological structure and immunohistochemical expression were observed under the light microscope,and relevant literature on its manifestations was reviewed.Results： The case was pathologically diagnosed as gray prostate tumor,with chondrosarcomatous and undifferentiated malignant mesenchymal components under the light microscope.Immunohistochemical staining revealed vimentin（＋）,local CD117（±）,SMA（-）,Des（-）,myoglobin（-）,CD34（-）,CK7（-）,and CK8（-）.Tumor metastasis was found 2 months after the operation,and the patient died 4 months later.Conclusion： Undifferentiated sarcoma of the prostate with cartilage metaplasia is a very rare and highly malignant aggressive tumor,which can be diagnosed by biopsy and immunohistochemistry.
