摘  要:目的探讨伴2型糖尿病的急性脑梗死(ACI)患者的颈内动脉狭窄的特点。方法对经颅多普勒或CT血管成像检查出颈内动脉狭窄的54例伴2型糖尿病的ACI患者进行数字减影血管造影(DSA)检查,并对结果进行分析。结果本组54例患者DSA检查均发现颈内动脉狭窄。单支血管狭窄发生率为42.6%(23/54),明显低于多支血管狭窄(57.4%,31/54)(P〈0.05)。颈内动脉重、中度狭窄的比例(20.3%,18.8%)明显低于轻度狭窄(53.9%),明显高于血管闭塞(7.0%)(均P〈0.05)。颈内动脉C4处狭窄的比例(18.8%)明显低于C1处(60.9%),明显高于C2、C3、C5、C6和C7处(1.6%~6.2%)(均P〈0.05)。结论伴2型糖尿病的ACI患者颈内动脉狭窄多为多支血管狭窄,狭窄程度以轻度为主,狭窄部位以C1狭窄最常见。Objective To study the features of internal carotid artery stenosis in acute cerebral infarction(ACI) patients with type 2 diabetes.Methods Fifty-four ACI patients with type 2 diabetes who were detected by transcranial doppler or CT angiography and found out internal carotid artery stenosis were examined by digital sabiractive angiography(DSA).And the results were analyzed.Results All of the 54 patients in this group were found internal carotid artery stenosis by DSA.The incidence of stenosis in single vessel was(42.6%,23/54),which was significantly lower than stenosis in multiple vessels(57.4%,31/54)(P0.05).The severe and moderate rate of internal carotid artery stenosis(20.3%,18.8%) were significantly lower than mild(53.9%),and were significantly higher than occlusion(7.0%)(all P0.05).The C4 rate of internal carotid artery stenosis(18.8%) was significantly lower than C1(60.9%),and was significantly higher than C2,C3,C5,C6 and C7(1.6%-6.2%)(all P0.05).Conclusions The internal carotid artery stenosises in type 2 diabetes patients with ACI are mostly multiple vessels,and the degrees are mostly mild,and the location is often in C1.
