摘 要:目的探讨应用计算机导航技术切除复杂累及股骨颈骨软骨瘤的可行性和适用性。方法复杂累及股骨颈骨软骨瘤患者5例,采用CT数据导航,采用Stryker公司的脊柱导航软件,术中导航指引定位、精确切除肿瘤,并保留股骨颈结构的完整。结果全部病例顺利完成导航手术,术后疼痛及活动受限缓解。经术野判定、术中导航指引器确认和术后X线平片确认手术边界充分、肿瘤去除彻底、股骨颈形态结构完整。术后组织病理学诊断均为骨软骨瘤。平均随访28.6个月,未见局部复发和病理骨折。结论计算机导航技术可术中实时指导手术切除入路、检验切除结果,从而达到复杂累及股骨颈骨软骨瘤的精确切除,避免了肿瘤的局部复发和股骨颈的病理骨折。Objective To report our experience for the precise resection of complicated osteochondroma of femoral neck using computer navigation system. Methods Surgical resections with computer navigation system were performed for 5 patients who had complicated osteochondroma of femoral neck. CT-based navigation was carried out in all cases. The nav- igation system software was Spine Navigation 1.2 by Stryker. The pointer was helpful to localize the lesion and to resect the lesion precisely without removal of any excess bone. Results All the computer navigation-guided excisions were finished successfully with patients' pain relief and movement amelioration postoperatively. The complete clearance of lesion and no impact of femoral neck structure were confirmed by intraoperative visual inspection and pointer confirmation, postoperative X-ray in all cases. All cases had histopathological diagnosis of osteochondroma. All cases were followed up for 28.6 months averagely and no local recurrence and pathological fracture occurred. Conclusions For complicated osteochondromas of femoral neck, the navigation system can show the excision direction and depth real-timely and is very helpful for the precise resection. Local recurrence and pathological fracture will be avoided effectively.
