连环蛋白p120在人胰腺癌组织中的表达及其意义

摘　　要：目的:了解胰腺癌组织中连环蛋白p120(p120ctn)及E-钙黏蛋白(E-cadherin)表达情况,分析它们与胰腺癌临床病理特征及预后的关系。方法:采用免疫组化技术检测53例胰腺癌及癌旁胰腺组织的p120ctn、E-cadherin和CD34表达情况,结合临床资料利用SPSS软件行统计学分析。结果:53例胰腺癌组织中,连环蛋白p120主要表现为胞膜表达减弱或消失,或细胞质表达,异常表达率为34/53(64.1%),显著高于癌旁胰腺组织(7/53),差异有统计学意义(P<0.05)。E-cadherin在胰腺癌中异常表达率为30/53(56.6%),高于癌旁组织(9/53),差异有统计学意义(P<0.05)。p120ctn的异常表达与E-cadherin异常表达呈正相关(r=0.457,P<0.05)。p120ctn异常表达组微血管密度(MVD)值明显高于正常表达组(49.29±6.86 vs 13.80±2.92,P<0.05)。p120ctn的异常表达与胰腺癌组织的分化程度、淋巴结转移及TNM分期呈明显相关(P<0.05)。p120ctn异常表达组患者中位生存时间[(12.9±2.5)个月]明显低于正常表达组[(25.0±5.7)个月](P<0.05)。结论:p120ctn在胰腺癌中存在异常表达,其异常表达不仅与E-cadherin、CD34表达有关,且与肿瘤分化程度、淋巴结转移及TNM分期密切相关,并影响患者的预后。p120ctn可能是潜在的评价胰腺癌预后的指标和治疗靶点。Objective： To examine the expression in vivo and cellular localization of p120-catenin（p120ctn） and E-cadherin in pancreatic carcinoma and to investigate the association of p120ctn expression with the clinicopathologic features and prognosis of pancreatic carcinoma.Methods： Immunohistochemistry was performed to investigate the expression of p120ctn protein,E-cadherin protein,CD34 in 53 cases with pancreatic carcinoma.The relationship between p120ctn protein expression and pathological characteristics and prognosis was also assessed.Results： Compared with the adjacent tissues,more reduced or loss of membranous expression and cytoplasmic localization of p120ctn was found in pancreatic carcinoma tissues（34/53,64.1%,P0.05）.E-cadherin protein expression（30/53,56.6%） was similar to that of p120ctn,additionally,there was a positive relationship between p120ctn protein and E-cadherin protein expression（P0.05） in pancreatic carcinoma tissues.Similarly,a positive correlation can be also found between the altered expression of p120ctn and MVD value.Not only did abnormal expression of p120ctn in pancreatic carcinoma tissues correlate with tumor differentiation,lymph node metastasis,TNM stage（P0.05）,but also with a poor prognosis（P0.05）.Conclusions： The p120ctn expression is altered and related to not only the expression of E-cadherin and CD34 protein,but also tumor differentiation,lymph node metastasis,TNM stage and prognosis of pancreatic carcinoma.It indicates that p120ctn may be a potential prognostic factor and therapy target of pancreatic carcinoma.
