摘 要:目的:建立内毒素休克大鼠模型,探讨内毒素休克形成机制。方法:将40只Wistar大鼠随机分为正常对照组(n=10)和内毒素休克组(n=30)。内毒素休克组采用腹腔注射适量D-氨基半乳糖(D-GalN)致敏后,经尾静脉注射休克剂量内毒素脂多糖(LPS),观察大鼠平均动脉压(MAP)变化,以评定模型是否成功。ELISA法测定血清中肿瘤坏死因子(TNF-α)、血清及肾上腺组织中LPS浓度,电化学放光免疫分析法测定血清中皮质醇浓度,光镜下观察肾上腺组织的形态学改变。结果:注射休克剂量的LPS后,大鼠MAP迅速下降,回升至正常后逐渐缓慢下降并维持较低水平,其中19只大鼠符合休克标准,其血清中TNF-α、皮质醇、LPS的质量浓度分别为842.41 ng.L-1、9.50μg.L-1、231.44μg.L-1,肾上腺组织中的LPS质量浓度为154.23μg.L-1。光镜下可见肾上腺组织有明显充血和水肿。结论:LPS、TNF-α参与了内毒素休克的发生、发展过程。休克大鼠的血清皮质醇浓度明显升高,并伴有肾上腺组织的病理改变。Objective: To detect serum TNF-α,corticosteroid,LPS level and adrenal glands tissue LPS level by producing the rat models with endotoxic shock and to observe histological changes of adrenal gland tissue with microscopy.Methods: Forty Wistar rats were randomly divided into two groups: normal control group(n=10),endotoxic shock group(n=30).Endotoxic shock group were sensitized by D-GalN by intraperitoneal injection,then given LPS by tail vein injection.Changes of mean arterial pressure(MAP) were observed to evaluate the animal model.Serum TNF-α concentration,serum and adrenal glands tissue LPS concentrations were measured by enzyme-linked immunosorbent assay(ELISA).Serum corticosteroid concentration was detected by electro-chemiluminescence immunoassay(ECLI).Histological alterations of adrenal glands tissue were studied with microscopy.Results: The MAP decreased obviously after injection of LPS,then recovered to normal,and decreased slowly,and kept at a low level.Nineteen rats corresponded shock standards,whose serum TNF-α,corticosteroid and LPS concentrations were respectively 842.41 ng·L-1,9.50 μg·L-1,231.44 μg·L-1,and adrenal glands tissue LPS concentration was 154.23 μg·L-1.Pathological changes of adrenal glands tissue presented congestion and dropsy.Conclusion: LPS,TNF-α involves in the genesis and development of endotoxic shock.Serum corticosteroid of endotoxic shock rats increases obviously accompanied by pathological changes of the adrenal glands tissue.
