摘　　要：目的:探讨结直肠癌患者血清中TNF-α水平与瘦素表达和肿瘤临床表现型、胰岛素抵抗之间的关系.方法:肿瘤组选择100名结直肠癌患者,选择80名健康成年人作为对照组.采用用双抗体夹心ABC-ELISA法测定血清TNF-α及瘦素水平.结果:肿瘤组血清TNF-α水平为(125.68pg/L)明显高于正常对照组(53.57pg/L)P=0.008TNF-α的表达与TNM分期相关,随着临床分期(TNM)增高而升高P<0.001.与组织学分级无明显关系(P=0.13).血清TNF-α水平与瘦素表达呈正相关(P=0.11<0.05),与HOMA-IR也呈明显正相关(P<0.001).结论:结直肠癌患者的血清中的TNF-α水平明显升高.并随着TNM分期的增加而升高,而与组织学分级无关.血清中TNF-α的水平与瘦素表达及HOMA-IR明显相关.AIM：To investigate the relationship of serum TNF-α level with serum leptin and insulin resistance in patients with colorectal carcinoma.METHODS：ELISA was used to detect serum levels of leptin and TNF-α in 100 patients with colorectal carcinoma and 80 normal controls.Fasting plasma glucose（FPG） and fasting serum insulin（FIns） were measured to calculate the ho-meostasis model assessment of insulin resistance（HOMA-IR）.The clinicopathological features were also recorded and analyzed.RESULTS：Serum TNF-α levels in patients with colorectal carcinoma were significantly higher than in normal controls（125.68 pg/L vs 53.57 pg/L,P 0.05）.There was a significant difference in serum TNF-α levels among patients with tumors of different stages（TNM）（P 0.001）.A significant correlation was noted among serum leptin,serum TNF-α and HOMA-IR in patients with colorectal carcinoma（P 0.001）.CONCLUSION：Serum TNF-α levels in patients with colorectal carcinoma were significantly higher than in normal persons.Tumor stage is associated with serum TNF-α levels in patients with colorectal carcinoma.There is a significant correlation among serum TNF-α,HOMA-IR and serum leptin in patients with colorectal carcinoma.
