内镜下尼龙绳套扎或金属钛夹钳夹联合高频电凝切除治疗大肠巨大息肉的临床应用  

High-frequency Electric Coagulation Resection Combined with Nylon Thread Ligation or Using Metal Titanium Clamp under Endoscope for Treating Giant Polyp of Large Intestine

摘　　要：为探讨内镜下尼龙绳套扎或金属钛夹钳夹联合高频电凝切除治疗大肠巨大息肉的疗效，对19例26枚大肠巨大息肉采用此方法治疗。结果显示，17枚亚蒂或有蒂息肉一次性尼龙绳套扎并切除成功，9枚长蒂息肉金属钛夹阻断血流后切除成功。术后无严重并发症发生。2周后内镜复查显示，14例创面愈合仅留瘢痕；3例创面处形成浅溃疡；1例创面处留有息肉样组织，追加热探头灼除；1例残端过长，追加尼龙绳套扎切除。结果表明，内镜下尼龙绳套扎或金属钛夹钳夹联合高频电凝切除治疗大肠巨大息肉安全有效，操作简单，并发症少。The objective of this study was to investigate the therapeutic effect of high-frequency electric coagulation resection combined with nylon thread ligation or metal titanium clamp management under endoscope in the treatment of giant polyp of large intestine. The 19 patients with this disease （total polyp＇s number were 26） received above-mentioned combined therapy. As results, 17 of pedicle or sub-pedicle polyps were ligated by using nylon thread and resected primarily and successfully; 9 of long pedicle polyps were resected through blocking blood flow by using metal titanium clamp;after operation no severe complication occurred;later 2 weeks re-examination under endoscope,wound surface of 14 cases was healing only with scar,that of 3 cases had superficial ulcer, that of 1 case left polyp-like tissue being then removed by hotprobe cauterization;in 1 case was again resected by using nylon thread ligation due to the too long residual end of polyp. Results show that above-mentioned combined therapy is safe and effective,and of simple manipulation and less complication, for treating giant polyp of large intestine.
