C4d在人类异体小肠移植术后肠黏膜中的表达及意义

摘　　要：目的研究小肠移植排异反应中C4d沉积的部位和意义。方法对小肠移植患者的肠活检标本做C4d免疫组化染色,观察C4d阳性沉积情况与小肠移植病理改变的关系。结果 C4d以沉积在血管内皮细胞和淋巴滤泡生发中心为主。C4d的阳性表达率与小肠急性排异反应的病理评分具有相关性,且随着评分的升高而升高(P<0.05)。非排异反应阶段与可疑急性排异反应阶段则无差别(P>0.05)。此外,在正常对照组和克罗恩肠病组中不表达。结论抗体介导的体液免疫反应可能在小肠移植中也是存在的,不仅为病理医师提供了一项参考依据,更为临床医师治疗小肠移植急性排异反应补充了新的途径。Purpose To investigate the location and significance of C4d deposition in small bowel with transplantation rejection.Methods Immunohistological staining was performed to observe C4d deposition in the small bowel allograft biopsies as well as the relationship with the pathological changes of small bowel.Results C4d expressed on the vessel and the germinal center of folliculus lymphaticus.The expression of C4d increased significantly as rejection severity increased（P0.05）.There is no significant difference between the non-rejection groups and indefinable groups（P0.05）.However,the express of C4d is negative in inflammatory bowel diseases and control group.Conclusion Antibody-mediated humoral immune reaction maybe occur in the small bowel transplantation rejection.Both of the pathologist and clinician could found valuable information in the expression of C4d.
