蛇毒降纤酶治疗外伤性玻璃体积血的实验研究

摘　　要：目的观察蛇毒降纤酶对实验性外伤性玻璃体积血的治疗效果。方法用20只新西兰大白兔右眼建立外伤性玻璃体积血的动物模型,随机分为生理盐水组和蛇毒降纤酶组,直接检眼镜分别观察两组玻璃体积血指数和增殖性玻璃体视网膜病变(proliferative vitreoretinopathy,PVR)程度;酶联免疫吸附法(enzyme-liked immunosorbent assay,ELISA)检测玻璃体液中各细胞因子浓度。结果蛇毒降纤酶组玻璃体积血指数、PVR程度、Ⅲ型胶原、基质金属蛋白9(matrix metallopro-teinase-9,MMP-9)浓度低于生理盐水组,两组分别比较差异均有统计学意义(P<0.05);白介素-1β、血小板源性生长因子、转化生长因子β1、表皮生长因子浓度与生理盐水组比较无统计学意义。结论蛇毒降纤酶能促进玻璃体积血的吸收、减轻外伤性PVR程度;并能下调玻璃体腔MMP-9、Ⅲ型胶原的浓度。ObjectiveTo study the therapeutic effect of defibrase on experimental traumatic vitreous hemorrhage.MethodsAll right eyes of 20 New Zealand albino rabbits were created as experimental rabbit model eyes of traumatic vitreous hemorrhage.The rabbits were divided into phosphate buffered saline group、defibrase group randomly.The bleeding indexes in vitreous and degree of proliferative vitreoretinopathy were estimated by direct ophthalmoscope examination.The content of cytokine in vitreous was detected by enzyme-liked immunosorbent assay.ResultsThe bleeding indexes,degrees of proliferative vitreoretinopathy,levels of collagen type Ⅲ and matrix metalloproteinase-9 in vitreous in defibrase group were lower than those of phosphate buffered saline group（P〈0.05）.ConclusionDefibrase can promote the absorption of vitreous hemorrhage,decrease the degrees of proliferative vitreoretinopathy and suppresse the production of MMP-9 in vitreous.
