彩色多普勒超声在辅助诊断多发性骨髓瘤中的应用价值

摘　　要：目的探讨彩色多普勒超声对多发性骨髓瘤的辅助诊断价值。方法对80例多发性骨髓瘤患者不同受累器官或部位彩色多普勒超声表现进行分析。结果 80例中73例骨骼受累,21例肾脏受累,5例肝脏肿大,10例脾脏肿大,6例淋巴结增大,15例胸腔积液,40例不同部位的髓外浆细胞瘤。其中不同部位髓外浆细胞瘤均表现为实性低回声包块,伴有或不伴有骨质破坏。结论彩色多普勒超声在多发性骨髓瘤多发器官浸润的辅助诊断中起着重要作用,可为临床诊断与鉴别诊断、治疗方案的制定提供重要影像学依据。Objective To assess the value of color Doppler ultrasonography in the diagnosis of mul- tiple myeloma （MM）. Methods A total of 80 patients with multiple myeloma were included. Their docu- ments of abdominal and pectoral ultrasonography, echocardiography, and high-frequency sonography in bone and subcutaneous mass were retrospectively analyzed. Results Sonographic features with internal low ech- oes （or with bone destructions） in the seventy-three cases of bone and cartilage destruction, 21 cases of re- nal damage, 5 cases of hepatomegaly, 10 cases of splenomegaly, 6 cases of lymphnode enlargement, 15 ca- ses of pleural effusion, and 40 cases of extramedullary plasmacytoma were found. Conclusions The color doppler uhrasonography is helpful to assess the organ infiltration of multiple myeloma, which could offer the reference for clinical and differential diagnosis.
