摘 要:目的 探讨磁共振梯度双回波化学位移成像定量诊断脂肪肝的临床应用。方法20例弥漫性脂肪肝病例,周期行CT及磁共振梯度双回波化学位移成像,分别对肝VI段及IV段(Couinaud法)选取同一部位感兴趣区进行测量。正反相位信号强度差/正相位信号强度(SIP—SOP)/SIP及正反相位信号强度差SIP—SOP与肝/脾CT值比值(L/S)进行Spearman相关性分析及建立直线同归方程。根据L/S评价脂肪肝程度的标准,计算(SIP—SOP)/sIP、SIP-SOP评价脂肪肝程度的标准。结果(SIP-SOP)/SIP、SIP-SOP与L/S的相关系数分别是r=-0.908及r=-844(P〈0.01),回归方程分别是y=-1.751x+1.278及Y=-1.s8×10-3x+1.005,(SIP-SOP)/sIP诊断脂肪肝的标准为:轻度0.329〉(SIP-SOP)/sIP≥0.159、中度0.444〉(SIP-SOP)/sIP≥0.329.重度为(SIP-SOP)/sIP≥0.444。SIP-SOP诊断脂肪肝的标准为轻度:193.0〉SIP—SOP≥3.2,中度:319.6〉sIP—SOP≥193.0.重度:sIP—sOP≥319.6。结论磁共振梯度双回波化学位移技术与CT定量诊断脂肪肝具有较好的相关性,可成为临床定量诊断脂肪肝的一种简单、有效方法。Objective To evaluate the clinical application of MR dual-echo chemical shift gradient-echo imaging in quantitative diagnosis of fatty liver. Methods 20 patients with diffused fatty liver underwent CT and MR dual-echo chemical shift gradient-echo imaging simutaneously. The liver segments (Couinaud} VI and IV were selected separately at region of interest (ROI) in the same site to study the relationship between( SIP-SOP )/SIP, SSIP-SOP and L/S ( SIP : signal intensity of in-phase, SOP : signal intensity of outphase, L/S: CT attenuation ratio of liver to spleen ) using Spearman correlation coefficient and linear regression equation. The diagnostic criteria of fatty liver using(SIP-SOP )/SIP and SIP- Sop were calculated refer to the criteria of L/S. Results (SIP-SOP )/SIP and SIP-SOP had significant relationship with L/S( r=-0. 908 and r=- 0. 844, P〈0.01 respectively), linear regression equations were y=1. 751x+ 1. 278 and y=-1.58× 10-3 x+1. 005 respectively. The diagnostic criteria of fatty liver for (SIP-SOP )/Sw were as following, mild level:0. 329〉 ( SIP- SoP )/SIP≥0. 159, moderate level: 0. 444〉 ( SIP- SOP )/SIP≥0. 329 and severe level: ( SIP -SOP )/SIP≥0. 444,and the diagnostic criteria for SIP-Sop were as following,mild level: 193.0〉SIp-Sop3.2,moderate level:319.6 % ( Sip- Sop ) ≥ 193.0 and severe level: ( SIp- Sop ) ≥319. 6. Conclusion MR dual-echo chemical shift gradient-echo imaging is of significant relationship with CT in quantitative diagnosis of fatty liver, it is an easy and effective method in clinical application in quantitative diagnosis of fatty liver.
