摘  要:目的:探讨血管内皮细胞生长因子(VEGF)与细胞黏附分子CD15的表达及其与骨肉瘤的分级、转移和预后的关系。方法:免疫组化法检测38例骨肉瘤组织中VEGF和CD15的表达水平,并结合骨肉瘤的生物学行为及临床随访资料进行分析和综合评价。同时设10例骨软骨瘤作为阴性对照组。结果:骨软骨瘤组织中VEGF的阳性率为20.0%(2/10),CD15呈阴性表达。骨肉瘤组织中VEGF和CD15的表达率分别是76.3%(29/38)和65.8%(25/38)。VEGF与病理分级无关,P=0.391;CD15与病理分级有关,P=0.028。VEGF和CD15都与转移有关,r值分别为0.612 2、0.695 7。CD15和(或)VEGF阳性表达者5年生存率均明显低于阴性表达组,P=0.001。CD15和VEGF均阳性表达者生存率最低,而二者均阴性表达者生存率最高。结论:VEGF和CD15是骨肉瘤转移和预后不良的重要生物学指标,两者在骨肉瘤的发生和转移方面具有相互协同作用。临床上,检测VEGF和CD15的表达对骨肉瘤患者的预后有重要价值。OBJECTIVE:To study the association of vascular endothelial growth factor(VEGF) and the expression of cell adhesion molecules CD15 in osteosarcoma grade,metastasis and prognosis.METHODS: Use immunohistochemical method to detect 38 cases of osteosarcoma in the expression of VEGF and CD15,combined with the biological behavior of osteosarcoma and clinical follow-up data for analysis and evaluation.At the same time with 10 cases of benign tumor of bone cartilage as a negative control.RESULTS: VEGF positive rate in benign tumor of bone was 20.0%(2/10),and CD15 was negative.VEGF in osteosarcoma and CD15 expression rates were 76.3%(29/38) and 65.8%(25/38).Expression of VEGF was not associated with tumor grade(P=0.391),but CD15 was(P=0.028).Both VEGF and CD15 were related to osteosarcoma metastasis(r= 0.612 2,r=0.695 7).The 5-year survival rate of patients with VEGF and(or) CD15 positive expression was significantly lower than those with negative expression(P=0.001).CONCLUSIONS: VEGF and CD15 are important biological indicators of metastasis and poor prognosis,both have a synergistic effect in the occurrence and metastasis of osteosarcoma.Clinically,detection of expression of VEGF and CD15 for the prognosis of osteosarcoma patients have significant value.
