原发性胆囊癌Flt-4的表达与肿瘤恶性特征的相关性分析

摘　　要：目的探讨原发性胆囊癌组织中Flt-4在mRNA和蛋白水平的表达及其与肿瘤恶性特征的关系。方法选取2000年8月至2010年8月我院手术切除病理明确的19例原发性胆囊癌和13例胆囊腺瘤样息肉新鲜组织标本,采用逆转录聚合酶链反应(RT-PCR)和免疫组化染色在mRNA和蛋白水平检测Flt-4在原发性胆囊癌和胆囊腺瘤样息肉的表达情况,并分析Flt-4免疫组化阳性表达的病例与胆囊癌肝浸润、淋巴转移、病理分级和Nevin分期等恶性特征的相关性。结果在胆囊癌和胆囊腺瘤样息肉组织中Flt-4 mRNA阳性表达率为73.68%(14/19)和7.69%(1/13);两组相比差异显著(P<0.05)。免疫组化染色Flt-4蛋白阳性表达率为68.42%(13/19)和7.69%(1/13);两组相比差异有统计意义(P<0.05)。Flt-4的阳性表达率在胆囊癌肝浸润、淋巴转移和Nevin IV、V期组明显高于无肝浸润、无淋巴转移和Nevin I、II、III期组(P<0.05),而在高、中和低分化组三者的阳性表达率无显著差异(P>0.05)。结论原发性胆囊癌组织中Flt-4在mRNA和蛋白水平均存在高表达,Flt-4的阳性表达同胆囊癌肝浸润、淋巴转移和Nevin分期明显相关,但与病理分级无明显相关。Objective To analyze the Flt-4 mRNA and protein expression in primary gallbladder carcinoma（PGC） and their correlation with clinical pathological characteristics of primary gallbladder carcinoma.Methods The fresh resected samples of 19 cases PGC and 13 cases gallbladder adenomaous polypus（GAP） which were finally diagnosed by pathology were collected from Aug.2000 to Aug.2010.The Flt-4 mRNA and protein expression of all cases were detected with RT-PCR and immunohistochemistry.The correlation between Flt-4 protein positive expression in PGC and the clinical pathological characteristics of PGC were analyzed as well.Results The Flt-4 mRNA positive expression rates were 73.68%（14/19） and 7.69%（1/13）,which had significant difference between PGC cases and GAP cases（P〈0.05）.The Flt-4 protein positive expression rates of all PGC and GAP cases were 68.42%（13/19） and 7.69%（1/13）,which also had significant difference between two groups（P〈0.05）.The Flt-4 protein positive expression rates in cases of PGC which had liver infiltration,lymphatic metastasis and Nevin IV and V stage were significantly higher than those had no liver infiltration,lymphatic metastasis and Nevin I,II and III stage of PGC（P〈0.05）.However,the positive expression rates of Flt-4 protein had no any difference among the high,middle and low differentiation of PGC cases（P〈0.05）.Conclusion The Flt-4 mRNA and protein are all over-expression in cases of PGC and the over-expressions of Flt-4 protein are significantly correlative with PGC liver infiltration,lymphatic metastasis and Nevin IV and V stage,but they have nothing to do with the PGC pathological differentiation.
