中年原发性闭角型青光眼患者心理因素分析

摘　　要：目的分析中年原发性闭角型青光眼患者的心理因素特点。方法对56例45～60岁的原发性闭角型青光眼患者(青光眼组)和51例45～60岁的正常人群(正常组)分别用汉米尔顿焦虑量表(HamiltonAnxiety Scale,HAMA),汉米尔顿抑郁量表(Hamilton Depression Rating Scale for Depression,HRSD)、躁狂评定量表(Mania Rating Scale,MRS)进行测评,记分项目包括:HAMA表总分A,包括2个因子,精神性焦虑(A1),躯体性焦虑(A2);HRSD表总分用B表示,包括5个因子,焦虑/躯体化(B1),体重(B2),认识障碍(B3),迟缓(B4),睡眠障碍(B5);MRS表总分用C表示,病情程度用病情指数(C0)表示。青光眼组(G)与正常人组(Z)各项得分进行统计学分析。结果所有受试者的各量表总分均在抑郁或焦虑症、躁狂状态的诊断分数以下。2组各项得分比较,有3项因子无统计学意义差异:B2G与B2Z(t=0.438,P=0.667>0.05),B3G与B3Z(t=2.051,P=0.055>0.05),B5G与B5Z(t=1.535,P=0.142>0.05)。其余各因子间差异均有统计学意义(P<0.05)。结论中年青光眼患者群在心理评定量表中的多项因子与正常人有明显差异,其特征性心理因素包括焦虑、想法固执,易激惹等。OBJECTIVE To analyse the psychological factors of the middle-aged patients with closed-angle glaucoma. METHODS Fifty-six middle-aged patients with primary closed-angle glaucoma and fifty-one control mid-dle-aged people （45-60 years old） were investigated with Hamilton Anxiety Scale （HAMA） and Hamilton Depres- sion Rating Scale for Depression （HRSD） and Mania Rating Scale （MRS）. RESULTS Comparing the scores of psy-chological factors of the two groups, the expected scores of the B2 factor in HRSD （t=0.438,P=0.667〉0.05）, B3 factor in HRSD （t=2.05 l, P=0.055〉0.05 ） and B5 factor in HRSD （t=1.535 ,P=O. 142〉0.05） were not significantly different, while the others were significantly different （P〈0.01, P〈0.05 ）. This result showed that the psychological characteristics varied clearly comparing the patients with primary closed-angle glaucoma and control middle-aged people （45-60 years old）. CONCLUSIONS The psychological characteristics varied largely comparing the scores of psy- chological factors of the two groups. And the psychological characteristics of the patients with primary closed-angle glaucoma were anxiety, stubborn ideas and irritability.
