奥曲肽联合TACE对肝癌患者肿瘤增殖和血清VEGF变化的影响

摘　　要：目的 研究奥曲肽联合经导管肝动脉栓塞化疗（TACE）对肝细胞肝癌患者血清血管内皮生长因子（VEGF）水平变化及疗效的影响.方法 51例肝癌患者分为2组,联合治疗组给予奥曲肽＋TACE,单纯TACE组仅给予TACE.分别于TACE前和TACE后1、7、14、21、28天检测血清AFP和VEGF水平.结果 2组TACE后血清AFP均显著降低,其中联合治疗组TACE后21、28天血清AFP水平又显著低于单纯TACE组.2组TACE后1天血清 VEGF水平较治疗前显著升高,TACE后7天则显著下降,至TACE后14天再次升高;2组TACE后21、28天血清 VEGF水平又再度下降,其中联合治疗组血清VEGF水平显著低于单纯TACE组.联合治疗组的部分缓解（PR）和总有效率（RR）显著高于单纯TACE组.结论 奥曲肽联合TACE较单纯TACE能更显著抑制肝癌增殖,其协同抗癌的部分作用可能是通过降低VEGF水平.Objective To investigate the effects of octreotide combined with transcathetre hepatic arterial chemoem-bolization （TACE） on tumor growth and serum vascular endothelial growth factor （VEGF） levels in patients with hepato-cellular carcinoma （HCC）. Methods 51 patients with HCC were divided into two groups： one group was treated with octreotide plus TACE （combined treatment）, the other were treated with TACE alone. Serum levels of AFP and VEGF were detected before and 1, 7, 14, 21 and 28 days after TACE. Results Serum AFP levels decreased in both groups following TACE. At 21 and 28 days after TACE, serum AFP levels in patients with combined treatment were lower than in TACE alone. Serum VEGF levels in patients of both groups one day after TACE were higher than those before the treatment, 7 d after TACE, serum VEGF levels decreased; serum VEGF levels at 14 d increased again. Serum VEGF levels decreased again in both groups at 21 and 28 days after TACE, serum VEGF levels in combined treatment group were low- er than in TACE alone. The partial response （PR）and the overall response rate （RR） in patients of combined treatment group were significantly higher than those in TACE alone group. Conclusion Compared with TACE alone, octreotide combined with TACE could more significantly inhibit the growth of HCC, and the synergistic effect may be partly attributive to the reduction of the level of VEGF.
