兜带式后巩膜加固术治疗病理性近视眼的临床疗效

摘　　要：目的观察兜带式后巩膜加固术治疗病理性近视眼的临床疗效。方法采用兜带式后巩膜加固术治疗35例（66只眼）病理性近视眼患者，通过术后2年临床随访，主要观察患者视力、屈光度、眼压、眼轴、眼底血流速等改变。并经统计学处理分析。结果在观察66只眼中，术后第1、3、6、12、24月最佳矫正视力与术前比较基本是稳中有升，且随着观察时间延长，视力有上升趋势；屈光度、眼压、眼轴与术前比较差异无统计学意义；通过后巩膜加固手术后视网膜中央动脉（CRA）、睫状后短动脉（SPCA）收缩峰值流速（PSV）增加，舒张期最小流速（EDV）减少，血流阻力指数（RI）降低，眼底血流状态得到改善。结论只要精细手术操作，规范手术流程，采用改良兜带式后巩膜加固术可以稳定病理性近视眼视力、屈光度、眼压，阻止眼轴延长，改善眼底血流状态，提高患者视功能。Objective To observe clinical effect of pocket-style sclera reinforcement surgery tor the treatment of pathologic myopia. Methods Pocket-style sclera reinforcement surgery after treatment of 35 patients （66 eyes） with pathological myopia, by follow-up after 2 years, mainly focus on visual acuity, refraction, intraocular pressure, axial length, changes in retinal blood flow velocity, and all the data were treated with statistical analysis and processing. Results Out of 66 eyes, after the observa- tion of the following months： 1, 3, 6, 12 and 24 postoperative best corrected visual acuity was im- proved compared with the preoperative eyes, and with the observation of prolonged upward trend in sight; refraction, eye pressure difference between the preoperative axial length was not significant; through the posterior sclera reinforcement surgery for Central retinal artery （CRA）, short posterior ciliary artery （SPCA） peak systolic velocity （PSV） increase in the minimum diastolic velocity （EDV） decreased, resistance index （RI） decreased, retinal blood flow was improved. Conclusions As long as the surgical procedure is precise and standard, using modified pocket-style sclera reinforcement surgery can stabilize vision for pathological myopia, refraction, intraocular pressure, prevent axial extension, improve the retinal blood flow and finally increase patients＇ visual function
