玻璃体腔药物注射联合玻璃体手术治疗脉络膜脱离型视网膜脱离的探讨

摘　　要：目的探讨术前玻璃体腔注射曲安奈德及healon联合玻璃体手术治疗脉络膜脱离型视网膜脱离的疗效及安全性。方法选择未经有效治疗的脉络膜脱离型视网膜脱离患者11例（11只眼），于手术前经睫状体平坦部向玻璃体腔内注入曲安奈德混悬液和healon，注药后观察眼压葡萄膜炎反应及脉络膜脱离消失情况，并于5日之内行玻璃体手术。结果注药后11只眼眼压均恢复良好，葡萄膜炎反应症状均不同程度减轻，10只眼脉络膜脱离眼于注药后5日内消失，所有病例均行玻璃体手术，术后11只眼均视力有不同程度提高，10只眼视网膜复位良好。结论术前玻璃体腔注射曲安奈德和healon联合玻璃体手术是一种安全有效地治疗脉络膜脱离型视网膜脱离的方法。Objective To investigate the effect of vitrectomy with preoperatively intravitreal drug-injection for treatment of retinal detachment with choroidal detachment. Methods Eleven cases （11 eyes） of retinal detachment with choroidal detachment were chosen and triamcinolone acetonide （TA） and healon were administrated via intravitreal injection through pars plana. The intraocular pres- sure, uveitis and choroidal reattacbanent condition were observed. All eyes underwent with vitrectomy within 5 days after intravitreal drug-injection. Results All eyes were demonstrated elevated intraocular pressure and alleviated uveitis. Choroidsal detachment disappeared in 10 eyes within 5 days after in- travitreal drug-injection, then all eyes underwent vitrectomy, the postoperation vision improved in 11 eyes, retinal reattachment was attained in 10 eyes. Conclusions Vitrectomy with preoperatively intrav- itreal injection of triamcinolone acetonide and healon is an effective and safe method to treat retinal detachment with choroidal detachment.
