Oct-2、Ki67蛋白在弥漫性大B细胞淋巴瘤亚型中的表达意义

摘　　要：目的研究Oct-2、Ki67蛋白在弥漫性大B细胞淋巴瘤生发中心型、非生发中心型两组亚型中的表达差异以及二者相互关系,探讨弥漫性大B细胞淋巴瘤诊断分型的辅助方法。方法选取弥漫性大B细胞淋巴瘤60例,分为2个亚型组:生发中心型组32例,非生发中心型组28例;采用Envision免疫组织化学法检测Oct-2蛋白、Ki67蛋白在这2组亚型淋巴瘤中的表达,比较分析Oct-2蛋白、Ki67蛋白在这两组亚型淋巴瘤中的表达差异以及二者之间的表达差异。结果 1)Oct-2在弥漫性大B细胞淋巴瘤生发中心型有23例阳性细胞表达率≥20%,占71.88%(23/32);非生发中心型有9例阳性细胞表达率≥20%,占32.14%(9/28),2组的表达差异有统计学意义(P=0.03)。2)Ki67在弥漫性大B细胞淋巴瘤生发中心型有25例阳性细胞表达率≥20%,占78.13%(25/32);非生发中心型有23例阳性细胞表达率≥20%,占82.14%(23/28),2组的表达差异无统计学意义(P=0.326)。结论 1)Oct-2在弥漫性大B细胞淋巴瘤生发中心型阳性细胞表达率高于非生发中心型,可为弥漫性大B细胞淋巴瘤分型提供一定的参考依据;2)Ki67在2组的表达无差异,不能作为弥漫性大B细胞淋巴瘤分型的依据;3)Oct-2与Ki67在弥漫性大B细胞淋巴瘤生发中心型中的表达无差异,而二者在非生发中心型中的表达有差异。Objective To investigate differences in the expression of Oct-2 and Ki67 protein in germinal center subtype and non-germinal center subtype of diffuse large B cell lymphoma（DLBCL）,and to explore effective indexes for judging different types of DLBCL.Methods Sixty cases of DLBCL were divided into germinal center subtype of DLBCL group（32 cases）and non-germinal center subtype of DLBCL group（28 cases）.The expression of Oct-2 and Ki67 protein in DLBCL was detected by immunohistochemistry and was compared between the two groups.Results The positive rate of Oct-2 expression（positive cells≥20%） was 71.88%（23/32） in germinal center subtype of DLBCL group and 32.14%（9/28） in non-germinal center subtype of DLBCL group,with a significant difference between the two groups（P=0.03）.The positive rate of Ki67 expression（positive cells≥20%） was 78.13%（25/32） in germinal center subtype of DLBCL group and 82.14%（23/28） in non-germinal center subtype of DLBCL group,with no significant difference between the two groups（P=0.326）.Conclusion The positive rate of Oct-2 expression in germinal center subtype of DLBCL is higher than that in non-germinal center subtype of DLBCL.Therefore,the detection of Oct-2,but not Ki67,may provide a basis for the judgment of different types of DLBCL.Furthermore,there is a significant difference between Oct-2 expression and Ki67 expression in non-germinal center subtype of DLBCL,but not in germinal center subtype of DLBCL.
