下斜方肌皮瓣修复口腔颌面部巨大缺损临床研究

摘　　要：目的:探讨下斜方肌皮瓣在口腔颌面部恶性肿瘤术后大型组织缺损修复重建中的作用。方法:回顾性分析我科应用下斜方肌皮瓣对口腔颌面部恶性肿瘤根治性切除后的缺损进行同期整复12例,其中舌癌7例,口底癌3例,颊癌2例,皮瓣最小为6cm×6cm,最大为11cm×10cm。结果:12例皮瓣术后全部存活,颌面部形态及功能恢复基本满意。结论:下斜方肌皮瓣面积大,血管蒂较长,柔软较薄,血运可靠,操作简便,适合修复口腔颌面部恶性肿瘤根治性手术后巨大缺损。Objective：To approach to the role of lower trapezius myocutaneous flap in large organizations defect reconstruction of oral and maxillofacial after malignant tumor surgery.Methods：12 cases performed the repairment with lower trapezius myocutaneous flap in our department were retrospectively analyzed,of which,7 patients were with tongue carcinoma,3 with mouth floor carcinoma, 2 with buccal cancer;and skin flap ranged from 6cm×6cm to 11cm×11cm.Results;All skin flaps survived completely after surgeries,and the maxillofacial morphology and functional recovery were satisfactory.Conclusion：The lower trapezius myocutaneous flaps are large in area,vascular pedicle are longer,blood supply are reliable,simple to operate.Besides,they are soft and thinner, suitable for repairing the large defect of mouth and maxilloface caused by radical tumor surgeries.
