腔内超声在中低位直肠癌诊断中的应用

摘　　要：目的：探讨经直肠腔内超声（TRUS）对中低位直肠癌术前分期的诊断价值及其与TNM分期的关系。方法：对53例经病理证实为直肠癌的患者术前进行TRUS检查，同时采用TNM分期标准进行分期，并与术后TNM分期进行对照。结果：53例直肠癌术前TRUS检查总的诊断准确率为69.9%，T1-T4期TRUS诊断准确性分别为100%、57.1%、79.4%及60.0% ；TRUS判断肠旁淋巴结转移的准确性为69.9%（37/53），特异性为74.3%（24/53）。结论：TRUS检查对于中低位直肠癌术前分期有较高的诊断准确性,有助于提高术前分期诊断准确性。Objective To investigate the diagnostic value of transrectal ultrasonography （TRUS） in preoperative staging of middle- low rectal carcinoma and the relationship between TRUS and TNM staging. Methods Fifty-three patients with pathologi-cally confirmed rectal carcinoma were detected by TRUS before surgery. The preoperative transrectal ultrasonographic staging of rectal carcinoma was performed using the TNM staging system. The preoperative staging results were then compared with those of postoperative pathologic staging. Results The overall accuracy rate of preoperative staging of rectal carcinoma by TRUS was 69.9%.The diagnostic accuracy in T1-T4-stage patients by TRUS was 100%,57.1%,79.4% and 60%. the accuracy rate of lymph node metastasis around the bowel by TRUS WAS 69.9%, the specificity was 74.3%.Conclusion TRUS has a high diagnostic accuracy in preoperative staging of middle- low rectal carcinoma. TRUS can improve the diagnostic accuracy of preoperative staging of rectal carcinoma.
