摘　　要：目的探讨鸦胆子油乳剂联合顺铂胸腔内注射治疗恶性胸腔积液的疗效。方法 90例肺癌伴恶性胸腔积液患者经胸腔闭式引流术排尽胸腔积液后被随机均分为三组,A组胸腔内注入鸦胆子油乳剂、顺铂和地塞米松针剂;B组注入鸦胆子油乳剂和地塞米松针剂;C组注入顺铂和地塞米松针剂。4周后观察近期疗效和功能状态评分。结果 A组总有效率为80.0%,高于B组的46.7%和C组的50.0%(P<0.05)。A组功能状态评分优于B、C组。结论鸦胆子油乳剂联合顺铂胸腔内注射治疗恶性胸腔积液近期疗效好,能明显改善患者生活质量。Objective To investigate the efficacy of intrapleural injection of brucea javanica oil emulsion（BJOE） combined with cisplatin for treating malignant pleural effusion.Methods Ninety lung cancer patients with malignant pleural effusion were equally randomized into three groups of A（intrapleurally injected BJOE combined with cisplatin and dexamethasone）,B（intrapleurally injected BJOE combined with dexamethasone） and C（intrapleurally injected cisplatin combined with dexamethasone） after drawn off malignant pleural effusion by closed thoracic drainage.The short-term efficacy and performance status were observed four weeks later.Results The effectiveness rate was 80.0% in group A,which was higher than 46.7% in group B or 50.0% in group C（P〈0.05）.Performance status in group A was better than that in groups of B and C.Conclusion Intrapleural injection of BJOE combined with cisplatin has a good short-term efficacy and can significantly improve the quality of life in the patients with malignant pleural effusion.
