准分子激光前弹力层下角膜磨镶术与准分子激光上皮瓣下角膜磨镶术治疗相对薄角膜近视的临床对比研究

摘　　要：目的探讨准分子激光前弹力层下角膜磨镶术(sub-Bowman keratomileusis,SBK)和准分子激光上皮瓣下角膜磨镶术(laser-assisted sub-epithelial keratomileusis,LASEK)矫治相对薄角膜近视的疗效。方法对62例(124眼)相对薄角膜近视患者行SBK或LASEK。其中30例(60眼)行SBK,32例(64眼)行LASEK。随访6个月,记录2组患者术后视力、残余屈光度、角膜上皮下雾状混浊(haze)情况,并进行统计学分析。结果术后早期LASEK组有轻度或中度不适感。2组术后角膜地形图检查均无圆锥角膜发生。术后1d、3d、7d SBK组平均视力为0.89±0.35、1.03±0.44、1.08±0.22,LASEK组平均视力为0.61±0.28、0.75±0.30、0.98±0.16,2组比较差异均有统计学意义(均为P<0.05),术后早期SBK组视力恢复快。术后1个月、3个月、6个月2组视力、残余屈光度比较差异均无统计学意义(均为P>0.05)。术后6个月时SBK组58眼(96.7%)达到了术前最佳矫正视力,LASEK组61眼(95.3%)达到了术前最佳矫正视力,2组比较差异无统计学意义(χ2=0.15,P=0.70)。2组术后最佳矫正视力无一例下降。LASEK组个别患者出现0.5级或1级haze,SBK组无haze产生。结论 SBK和LASEK矫正薄角膜近视均安全、有效,SBK在早期视力恢复和避免haze产生方面优于LASEK。Objective To evaluate the efficacy of sub-Bowman keratomileusis（SBK）and laser-assisted sub-epithelial keratomileusis（LASEK） for correcting myopia with relative thin cornea.Methods Sixty-two patients（124 eyes）underwent SBK or LASEK surgery for correcting myopia with relative thin cornea,including 30 patients（60 eyes）for SBK and 32 patients（64 eyes）for LASEK.Visual acuity,remained diopter and corneal haze were examined after operation.The results were analyzed statistically.Results Patients had different degree of uncomfortable feeling early time after LASEK.Both groups showed no keratoconus by corneal topography examination after operation.Visual acuities were 0.89±0.35,1.03±0.44,1.08±0.22 at 1 day,3 days,7 days after SBK,while 0.61±0.28,0.75±0.30,0.98±0.16 at 1 day,3 days,7 days after LASEK,there were statistical differences（all P〈0.05）.Recovery of visual acuity in SBK group was quicker than that in LASEK group early time after operation.There was no statistical difference of visual acuity and remained diopter between SBK group and LASEK group at 1 month,3 months and 6 months postoperatively（all P〉0.05）.Visual acuity of 58 eyes（96.7%） at 6 months after SBK reached the best corrected visual acuity before operation,while 61 eyes（95.3%）after LASEK.There was no statistical difference between two groups（χ2=0.15,P=0.70）.Best corrected visual acuity remained the same to or more than that before operation in all operative eyes.0.5 and 1 degree of haze was found in several patients after LASEK.Haze was not found in SBK group.Conclusion SBK and LASEK are both safe and effective for correcting myopia with relative thin cornea.SBK is superior to LASEK in respect of visual acuity recovery at early time and avoiding haze.
