LASIK手术对近视患者角膜内皮细胞的影响

摘　　要：目的探讨准分子激光角膜原位磨镶术(laser in situ keratomileusis,LASIK)对近视患者角膜内皮细胞的影响。方法选取行LASIK手术的近视患者403例(806眼),分别于术前和术后3个月、6个月、12个月进行角膜内皮计检查,观察角膜内皮细胞密度、变异系数、六角形细胞百分比、平均面积的变化。结果术前角膜内皮细胞密度为(3102±147)mm-2,变异系数为41.217±0.413,六角形细胞百分比为(51.640±0.813)%,角膜内皮细胞平均面积为(376.5±24.3)μm2。术后3个月、6个月、12个月角膜内皮细胞的密度分别为(3074±131)mm-2、(3059±267)mm-2、(3108±291)mm-2,变异系数分别为41.959±0.677、43.851±0.756、43.586±0.863,六角形细胞百分比分别为(50.944±1.021)%、(51.294±0.981)%、(50.539±1.375)%,角膜内皮细胞平均面积分别为(368.1±23.5)μm2、(381.3±28.2)μm2、(379.1±27.6)μm2。LASIK手术前后及术后各时间点角膜内皮细胞密度、变异系数、六角形细胞百分比、平均面积差异均无统计学意义(均为P>0.05)。结论 LASIK手术对近视患者角膜内皮细胞无影响,但对角膜内皮细胞的"愈合储备"有无影响,是否会加速角膜内皮细胞凋亡的速度,还需更长时间的观察。Objective To explore the effect of laser in situ keratomileusis（LASIK） on corneal endothelial cells in myopic patients.Methods Four hundreds and three myopic patients（806 eyes） underwent LASIK.The corneal endothelial cell density,variation coefficient,percentage of hexagonal cells and average area were examined under the microscope before operation and 3 months,6 months and 12 months after operation.Results The average corneal endothelial cell densities were（3 102 ±147）mm-2,（3 074±131）mm-2,（3 059±267）mm-2 and（3 108±291） mm-2 before operation and 3 months,6 months and 12 months after operation,the variation coefficients were 41.217±0.413,41.959±0.677,43.851±0.756 and 43.586±0.863,the percentages of hexagonal cells were（51.640±0.813）%,（50.944±1.021）%,（51.294±0.981）% and（50.539±1.375）%,the average areas of corneal endothelial cell were（376.5±24.3）μm2,（368.1±23.5）μm2,（381.3±28.2）μm2 and（379.1±27.6）μm2,respectively.There was no statistical difference in above parameters between pre-operation and post-operation（all P〉0.05）.Conclusion LASIK for myopia has no effect on corneal endothelial cells,but whether will it speed up the corneal endothelial cells apoptosis and has effect on healing reserving of corneal endothelium cells are necessary to perform a long-term follow up.
