鸦胆子油乳注射液联合第三代含铂化疗方案治疗晚期非小细胞肺癌的Meta分析

摘　　要：目的系统评价鸦胆子油乳注射液联合第三代含铂化疗方案治疗晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)的临床疗效和安全性。方法检索Cochrane Library、Pubmed、EMBASE、CancerLit、中国生物医学文献等相关数据库,纳入鸦胆子油乳注射液联合第三代含铂化疗方案治疗NSCLC的随机对照试验(randomized controlled trials,RCTs)。结果共纳入7篇RCTs,共483位患者。Meta分析结果显示:与单纯第三代含铂化疗方案相比,鸦胆子油乳注射液联合第三代含铂化疗方案未显著提高有效率,但可提高临床受益率(RR=2.37,95%CI:1.42-3.95,P=0.001)及化疗后的Karnofsky评分(RR=2.29,95%CI:1.74-3.02,P=0.000 01),并可降低白细胞减低和消化道反应的发生率。可显著提高化疗后外周血CD3、CD4阳性、NK细胞百分比,并显著提高化疗后外周血CD4/CD8比例,未显著提高化疗后外周血CD8阳性细胞百分比。结论鸦胆子油乳注射液联合第三代含铂化疗方案较单用第三代含铂化疗方案可增加临床受益率、减轻血液及胃肠不良反应,具有调节免疫的作用。Objective To evaluate the clinical efficacy and safety of combined brucea javanica oil emulsion injection and third generation chemotherapy regimen containing platinum for advanced non-small cell lung cancer（NSCLC）.Methods Papers on relevant randomized controlled trials（RCT） involving combined brucea javanica oil emulsion injection and third generation chemotherapy regimen containing platinum for advanced NSCLC were searched from Cochrane library,PubMed,EMBase,CancerLit,VIP,CBM and CNKI.Results Seven papers on RCT involving 483 patients were included.Meta-analysis showed that combined brucea javanica oil emulsion injection and third generation chemotherapy regimen containing platinum did not have a significant better efficacy for advanced NSCLC than the third generation chemotherapy regimen containing platinum,but could significantly improve the clinical benefit response（RR=2.37,95%CI： 1.42-3.95,P=0.001）,Karnofsky score（RR=2.29,95%CI： 1.74-3.02,P=0.000 01） of advanced NSCLC,decrease the incidence of leukopenia and gastrointestinal reactions,and increase the percentage of peripheral blood positive CD3,CD4,NK cells and the CD4/CD8 ratio,but could not increase the percentage of peripheral blood positive CD8 cells.Conclusion Combined brucea javanica oil emulsion injection and third generation chemotherapy regimen containing platinum has a better clinical efficacy than simple third generation chemotherapy regimen containing platinum for advanced NSCLC,with less blood and gastrointestinal adverse effects,and plays a role in regulation of immunological function.
