视网膜母细胞瘤组织中Ki67,PTEN和survivin的表达及临床意义

摘　　要：目的 研究Ki67基因、张力蛋白同源基因（PTEN）和存活素（survivin）在视网膜母细胞瘤（RB）中表达及临床意义.方法 收集RB 44例和正常视网膜组织10例,采用免疫组织化学方法检测Ki67,PTEN和survivin的表达,并分析它们的表达与RB患者性别、临床分期、视神经受侵犯情况和分化程度之间关系.结果 在正常视网膜组织中Ki67和survivin表达均为阴性,PTEN表达均为阳性.在RB组织中,Ki67,PTEN和survivin的阳性表达率分别为47.7%（21/44）,65.9%（29/44）和59.1%（26/44）,PTEN表达明显降低,Ki67和survivin表达明显增高,与对照组比较差异有统计学意义（P＜0.05）.不同性别之间Ki67,PTEN和survivin表达差异均无统计学意义（P＞0.05）.眼内生长期和青光眼期（眼内压增高）Ki67和PTEN表达阳性率均较眼外期表达增高（P〈0.05）,但survivin表达阳性率则降低（P〈0.05）.无视神经浸润（N0）者Ki67和PTEN表达阳性率均高于N1~N3（P〈0.05）,survivin表达阳性率则降低（P〈0.05）.RB组织细胞分化为R0者,Ki67,PTEN和survivin表达阳性率明显高于R1和R2（P〈0.05）.结论 在RB组织中PTEN的阳性表达率显著降低,而Ki67和survivin阳性表达率显著高于正常视网膜组织.提示测定Ki67,PTEN和survivin表达对研究RB的发生发展、侵袭和转移有一定参考价值.Objective To study the expressions of Ki67,PTEN and survivin in retinoblastoma （RB） tissue and discuss their clinleal significance. Methods The expressions of Ki67,PTEN and survivin were detected by immunohistochemistry in 44 cases of RB slice and 10 normal retinal tissues,and analysed their relation with gender,clinical stage,optic nerve invasion and differentiation degree in the patients with RB. Results The expressions of Ki67 and survivin were negative,and that of PTEN was positive in all normal retinal tissues. In the RB tissue,positive expression rates of Ki67,PTEN and survivin were 47.7% （21/44）,65.9% （29/44） and 59.1% （26/44）,respectively. PTEN expression was significantly reduced;Ki67 and survlvin expressions were significantly increased,compared with control group, the difference was statistically signifi- cant （P〈0. 05）. The expressions of Ki67,PTEN and survivin had no significant difference between the sexes （P〉0.05）. In intraocular growing and glaucoma （intraoeular pressure rise） stages,the expressions of Ki67 and PTEN were increasing,and survivin was reducing,compared with that extraocular extension stage in RB patients （P〈0.05）. In RB patients with out optic nerve invasion （NO）,the expressions of Ki67 and PTEN were higher （P〈0.05）,and survivin expression was lower （P〈0. 05） than that associated with optic nerve invasion （N1-N3）. In RB cell differentiate,the expressions of Ki67,PTEN and survivin in R0 were significantly higher than that those in R1 and R2 （P〈0.05）. Conclusion In RB patients,positive expression of PTEN was significantly reduced,and the expressions of Ki67 and survivin were significantly higher than that in the normal retinal tissue. The expressions of Ki67 ,PTEN and survivin detected have important value in the development,invasion and metastasis in RB patients.
