近视眼LASIK术后AC／A值的变化及相关因素分析

摘　　要：目的探讨近视眼患者准分子激光原位角膜磨镶术（LASIK）后调节性集合（AC）与调节（A）比值的变化及相关因素分析。方法分别选取41例戴框架眼镜患者（框架眼镜组）和14例戴隐形眼镜患者（隐形眼镜组），采用同视机测定比较其LASIK术前戴镜和术后3个月裸眼AC／A值，并分析术后AC／A值与其相关因素（年龄、主视眼角膜厚度、术前角膜曲率、术前等效屈光度及术前AC／A值）的相关性。结果手术前后两组患者AC／A均呈正态分布，术后12例发生了主视眼的改变。两组患者术后AC／A值与术前比较，差异均无统计学意义（均P〉0．05）；两组间比较，手术前后AC／A值的差异亦无统计学意义（均P〉0．05）。LASIK术后裸眼AC／A仅与年龄呈明显负相关（P〈0．05），而与角膜厚度、术前角膜曲率、术前等效屈光度、术前AC／A值无明显相关性；仅框架眼镜组LASIK术后裸眼AC／A值与术前AC／A值明显相关（P〈0．05），其他均无相关性。结论对于长期戴镜者，无论是隐形眼镜还是框架眼镜，AC／A值均不会发生明显改变。LASIK术后3个月裸眼AC／A值基本恢复到术前戴镜水平，术后AC／A值仅与患者年龄呈负相关。Objective To investigate changes of accommodative convergence/accommodative （AC/A） ratio of myopia af- ter excimer laser in-situ keratomileusis （LASIK）. Methods Fifty five myopia patients （110 eyes） treated by LASIK were divided into two groups： patients wearing frame glasses（Gg） and patients wearing contact lens（Gc）. The AC/A ratios were measured with synoptohore before surgery（wearing glasses） and 3 months after surgery （naked eyes）. The correlation of postoperative AC/A ratio with related factors （age, corneal thickness of dominant eye, preoperative corneal curvature, preoperative equivalent diopter, preoperative AC/A ratio） was analyzed. Results AC/A ratio showed normal distribution in both Gg group and Gc group, before operation and after operation. There were no significant differences between preoperative and postoperative AC/A ratios both in Gg group（t=0.953,P=0.346） and in Gc group（t=0.299, P= 0.770）. There were no significant differences between Gg and Gc groups both in preoperative AC/C ratio （t=-0.289,P=0.776） and postoperative AC/A ratio（t=-0.591,P=0.560）. The postoperative AC/A ratio was negatively correlated with the age of patients; while the postoperative AC/A ratio was correlated with the preoper- ative AC/A ratio only in Gg group. Conclusion Postoperative AC/A ratios of naked eyes regain their preoperative levels with glasses 3 month after LASIK operation, both for those wearincl frame alasses or those wearina contact lens.
