义眼座植入及全厚皮片移植眼窝再造术

摘　　要：目的探讨无眼球重度眼窝狭窄行义眼座植入及全厚皮片移植眼窝再造术的效果。方法8例8眼无眼球重度眼窝狭窄，行一期或二期羟基磷灰石义眼座植入术。术后3—8个月，行全厚皮片移植眼窝再造术。结果随访6个月至3年。7例义眼座位置良好，1例感染而取出。7例皮片完全存活，1例上穹窿部皮片坏死，形成新的上穹窿狭窄，3个月后再次行全厚皮片移植上穹窿成形术，术后效果良好。结论无眼球的重度眼窝狭窄患者行羟基磷灰石义眼座植入术后，二期行全厚皮片眼窝再造术，取材来源广泛，操作简便，术后效果良好。Objective To discuss the clinical value of eye socket reconstruction with hydroxyapatite implant and full-thickness skin graft for anophtbalmia of serous conjunctival sac contrachion. Methods Eight cases （8 eyes） of severe conjunctival sac contraction with anophthalmia were studied, the treatment was divided into two stages：in the first stage, hydroxyapatite implant was implanted. And 3 -8 months later, in the second stage, the socket were reconstructed with full-thickness skin graft. Results Fowllowing up for 6 months to 3 years, HA implant in 7 cases bad a good position, but that in 1 case was removed because of bacterial infection; The full-thickness skin flaps were transferred successfully in 7 cases, and 3 months later a repeat full-thickness skin graft was operated, on the contracted fornix conjunctive superior where was suffered from necrosis of distal skin flap in 1 case, and the satisfactory clinical outcomes was obtained. Conclusion HA implant in first-stage and full-thickness skin graft in second-stage was a effective, accessible source and easy operation in treatment of severe contracted socket combine with anophthalmia.
