肝细胞癌门静脉癌栓致门静脉海绵样变性侧支循环的影像特征探讨

摘　　要：目的探讨肝细胞癌门静脉癌栓所致门静脉海绵样变侧支循环血管的构成及螺旋CT影像特征。方法对我院行上腹部螺旋CT双期增强扫描并由组织病理学证实或临床随访证实为肝细胞癌CT显示肝细胞癌侵犯门静脉主干并继发门静脉海绵样变性(CTPV)者的37例进行分析。结果 20例门静脉左支受累;其中13例累及门静脉主干;23例显示扩张、扭曲呈锯齿状明显强化的胆囊静脉;15例显示胃底周围增粗及迂曲强化的门静脉胃支;18例显示胰十二指肠沟内增粗的胰十二指肠后上静脉。结论螺旋CT增强扫描能清楚地显示CTPV主要侧支循环血管的构成、走行及分布特点;熟悉与重视这些侧支血管的解剖位置和影像学表现特征将会为临床诊断和治疗提供重要的信息。Objective To analyze the imagery character of collateral circulation by cavernous transformation of portal vein induced by Hepatocellular carcinoma with tumor thrombi.Methods 36 patients with collateral circulation by cavernous transformation of portal vein induced by Hepatocellular carcinoma with tumor thrombi were analyzed.Results There were 20 cases with lft branch of portal vein involvement,13 cases involving the portal vein,23 cases with expansion,distort the cystic vein of jagged significantly strengthened,15 cases with gastric augmentation and tortuosity around strengthening the stomach in portal vein support and 18 cases with pancreaticoduodenal trench on augmentation after pancreaticoduodenal veins.Conclusions Enhanced spiral CT scan clearly shows CTPV main collateral circulation composition,traveling and distributions of the vascular;familiar with and value these anatomic and imaging manifestations of collateral vessels feature will provide important information for clinical diagnosis and treatment.
