偏侧咀嚼患者颞下颌关节形态变化研究

摘　　要：目的探讨偏侧咀嚼对颞下颌关节的影响。方法通过拍摄颞下颌关节X线侧位片,测量56例1999—2001年就诊于四川大学华西口腔医院颞下颌关节诊室的偏侧咀嚼患者的关节窝深度、宽度以及髁道斜度,了解其颞下颌关节形态变化。结果30例非习惯侧出现关节结节后斜面骨质粗糙、关节窝骨皮质吸收变薄、髁状突功能面骨质不光滑样骨质改变(53.6%),21例习惯侧出现骨质改变(37.5%),两者比较差异有统计学意义(P<0.05)。偏侧咀嚼患者习惯侧与非习惯侧的颞下颌关节窝深度和髁道斜度之间差异也有统计学意义(P均<0.05),而两者间关节窝宽度差异无统计学意义(P>0.05)。结论偏侧咀嚼影响颞下颌关节形态。Objective To explore the effect of unilateral masticatory on temporomandibular joint.Methods X-ray films of 56 patients who were diagnosed as unilateral masticatory in Hua'xi Stomotological Hospital from 1999—2001 were studied.Among them,32 cases were male!and 24 cases were female.Their age ranged from 27 to 49 years old,averaged 35.8 years old.The depth and width of glenoid fossa and the declination of articular eminence were measured and the osseous changes were observed.Results Statistically significant difference between the habitual and unhabitual joint were found for osseous change;30 cases showed osseous change in unhabitual sides(53.6%)including coarse osseous of posterior slopes of the articular eminence and absorptive changes of the articular fossa,and 21 cases showed osseous change in habitual sides.There were also significant differences between habitual and unhabitual sides of the articular fossa depth(P < 0.01)and the decilation of articular eminence(P< 0.05).There was no significance in the width of glenoid fossa(P> 0.05).Conclusion Unilateral masticatory affects the morphous of temporomandibular joint.
