胃肠安对大肠癌术后化疗后患者调节性T细胞及生存质量的影响

摘　　要：目的观察胃肠安对大肠癌术后化疗后患者调节性T细胞及生存质量的影响。方法将84例大肠癌术后化疗后患者随机分为中药组、西药组及综合组,每组28例;中药组口服健脾中药胃肠安,西药组采用胸腺肽α1皮下注射,综合组予口服汤药和皮下注射胸腺肽α1。各组均治疗2个月,观察治疗前后患者调节性T细胞(regulatory T cell,Treg)、脾虚证候积分、体力状况(KPS评分)变化情况。结果①治疗后中药组及综合组脾虚证候积分明显下降(P<0.05),西药组无明显改善(P>0.05);②中药组及综合组治疗后KPS积分明显上升(P<0.05),西药组无明显变化(P>0.05);③综合组治疗后Treg细胞水平明显降低(P<0.05),中药组及西药组无明显变化(P>0.05);④组间治疗后比较,中药组、综合组较西药组脾虚证候积分、KPS积分差异有统计学意义(P<0.05),综合组较西药组Treg水平差异有统计学意义(P<0.05)。结论①胃肠安可以下调Treg水平,改善大肠癌术后化疗后患者低下的免疫功能;②胃肠安可以改善大肠癌术后化疗后患者的脾虚症状,提高生存质量。Objective To observe the effects of ＂Weichang＇ an Decoction＂ on regulatory T-cell and quality of life in postoperative and chemotherapeutic patients with cancer of large intestine. Methods Eighty-four subjects were randomly divided into three groups, twenty-eight patients in each group. TCM group received ＂Weichang＇ an Decoction＂, control group received Thymosin-cd and combination group received both of them, with the course of 2 months. Regulatory T-cell, scores of spleen-deficiency syndrome and physical fitness were observed before and after treatment. Results ①After two-month treatments, the scores of spleen deficiency syndrome were reduced significantly in TCM group and combination group（ P 〈0.05 ）and insignificantly in control group（ P 〉0.05 ）. ②Kamofsky performance status was increased significantly in TCM group and combination group（P 〈0.05）, and insignificantly in control group（ P 〉0.05 ）. ③T-cell was decreased significantly in combination group（P 〈0.05 ） , but insignificantly in TCM group and control group （P 〉0.05）. Conclusion ＂Weichang＇ an Decoction＂ may inhibit regulatory T-ceil in postoperative patients with cancer of large intestine and improve the life quality and spleen deficiency syndrome in postoperative patients with cancer of large intestine.
