垂体腺瘤术后低钠血症与血浆心房利钠多肽、醛固酮的关系

摘　　要：背景与目的:垂体腺瘤术后常并发电解质紊乱,其发病机制尚不清楚。本研究探讨血浆心房利钠多肽(ANP)、醛固酮(ALD)在垂体腺瘤患者术后发生低钠血症中的作用。方法:采用放射免疫法检测32例垂体腺瘤患者术前和术后第1、3、5、7天的血浆ANP、ALD的水平。结果:垂体腺瘤患者术后ANP明显增高,术后第1、3天与术前的差异具有统计学意义(P<0.05)。ALD术后逐渐下降,至第5天降至最低值。低钠血症组患者血浆ANP水平在术后第1、3、5天明显高于非低钠血症组患者,有统计学意义;而两组血浆ALD水平在术后第1、3、5、7天的差异均没有统计学意义。结论:ANP、ALD是影响垂体腺瘤术后低钠血症的重要因素。低钠血症组血浆ANP水平增加显著,是促进低钠血症形成的主要原因。BACKGROUND & OBJECTIVES:Electrolyte disturbance is a common post-operative complication of pituitary adenoma. The reason is still not fully understood. In this study, we investigated the role of ANP and ALD in hyponatremia after transsphenoidal surgery of pituitary adenomas. METHODS:The level of ANP and ALD in 32 cases with pituitary adenomas preoperatively and in the day 1, day 3, day 5 and day 7 after operation was measured by radioimmunoassay. RESULTS:The level of plasma ANP increased significantly after operation. The level of plasma ANP in all patients in day 1 and day 3 after operation increased significantly compared with the pre- operative level (P< 0.05). the first and the third day after operation was obviously different from that of preoperative, which had statistical significance (P< 0.05). The level of plasma ALD decreased gradually, and was lowest in day 5 after operation. The level of plasma ANP of patients with hyponatremia in day 1, day 3 and day 5 after operation was statistically different from that of patients without hyponatremia. No difference was found between the level of ALD in patients with and without hyponatremia. CONCLUSIONS: The level of plasma ANP and ALD changes significantly after the surgery of pituitary adenoma, which suggests that ANP and ALD may be important factors in hyponatremia. The increased level of plasma ANP after operation plays important role in hyponatremia.
