青光眼术后经透明角膜區白内障超聲乳化吸出术

摘　　要：目的 介紹青光眼術后白内障經透明角膜切口行白内障超聲乳化吸出并人工晶體植入術的方法和療效觀察。方法避開原有效濾過區象限選擇鼻上或顥侧透明角膜區,行白内障超聲乳化吸出并人工晶體植入術,觀察術后視力、眼壓、角膜内皮細胞變化及術后并發症情况。結果 83只眼眼壓術前術后對比,無統計毕差异(t檢驗P>0.05)。27％病例術后視力達0.5以上,角膜内皮細胞損失率為14.5％,術后普遍存在虹膜色素脱0、角膜后彈力層皺褶、前房炎症反應等并發症。結論 經透明角膜切口超聲乳化術治療青光眼術后白内障,其療效安全、可靠。Objective We reported the clinical results of cataract phacoemulsification and lens implantation through transparent corneal incison after glaucoma surgery. Methods Cataract phacoemulsification and IOL implantation was performed with avoidance of previous filtering bulb. The changes of vision, intraocular pressure (IOP), endothelia and postoperative complication were observed. Results The IOP had no significant changes. The visual acuity of 27% eyes increased to > 0.5and the rate of endothelial cells loss was 14. 5%. Postoperative complication included iridal depigmentation, ruffed corneal elastic layer, inflammatory reaction of anterior segment. Conclusion Phacoemulsifiction through frans parent corneal incision is effective and safe for cataract after glaucoma surgery.
