摘　　要：目的:探讨重度磨耗牙垂直距离的变化与固定修复方法的选择。方法:对65例129颗重度磨耗牙,根据垂直距离有无变化,分为三类修复:①重度磨耗牙,垂直距离未减小,牙合龈距离尚能进行常规固定修复;②重度磨耗牙,垂直距离未减小,牙合龈距离不足进行常规固定修复;③重度磨耗牙伴有垂直距离减小,牙合龈距离不足进行固定修复,须升高垂直距离。追踪观察1-7年,其中56例为两年以上。结果:65例病例中有63例修复后牙体、牙髓、牙周基本健康,修复体无脱落、无破损;咀嚼功能、美观改善均取得了较高的满意度,未出现颞下颌关节问题或未加重。结论:根据重度磨耗牙垂直距离有无变化进行固定修复方法选择,临床思路明确,便于临床医师掌握,在临床是可行的方法。Objective： To investigate the changes of occlusal vertical dimension（OVD） and the selection of fixed restorations for dental severe attrition.Methods： Sixth-five patients with one hundred twenty-nine severe attrition teeth were divided into three classification restorations according to the different changes of OVD： ①The OVD of patients with dental severe attrition was not decreased,and occlusal-gingival distance was available for restoration by routine fixed prosthesis;②The OVD of patients with dental severe attrition was not decreased,and occlusal-gingival distance was not available for restoration by routine fixed prosthesis;③The OVD of patients with dental severe attrition was decreased,and occlusal-gingival distance was not available for restoration by routine fixed prosthesis;the OVD must be increased.Follow-up period was from 1 year to 7 years,and 56 cases were followed up for over 2 years.Results： After the follow-up period,the situations of restored teeth,pulp,periodontal tissue and prostheses of the teeth in the 63 cases among the 65 cases were fine.The masticatory function of most patients was improved significantly with satisfied esthetic result.Temporomandibular joint disorder was not found or not become worse.Conclusion： It is feasible to use fixed prosthesis for dental severe attrition according to the change of occlusal vertical dimension.
