两种颈部形态种植体周围非负荷期骨吸收的对比研究

摘　　要：目的:研究不同颈部形态种植体周围非负荷期骨吸收程度,为合理使用不同颈部形态的种植体提供参考依据。方法:收集采用种植修复治疗的患者45例,共51枚颈部光滑的种植修复体,其中ITI组颈部呈杯形种植体27枚;Anthogyr组圆柱形24枚。测量所有患者3个月时颈部周围骨组织总吸收量和术后10天、1个月、2个月、3个月时两组各自的平均骨吸收量。用PLANMECA-X线牙片机以同一条件拍摄X线片,将X线片扫描到计算机,利用Photoshop8.0软件中的标尺工具进行测量。测得数据用SPSS13.0软件进行分析比较。结果:种植体植入10天、1个月、2个月和3个月后相应骨吸收总高度:ITI组分别为1.350±0.321mm、1.787±0.354mm、1.898±0.366mm和1.951±0.385mm;Anthogyr组分别为0.627±0.401mm、0.902±0.496mm、0.977±0.501mm、1.025±0.511mm。P=0.002<0.05;3个月末即非负荷期,种植体周围骨吸收的高度结果经比较P=0.000<0.05;10天时ITI组骨吸收高度大于Anthogyr组,1个月、2个月和3个月时的两组组间的总吸收高度相比,差异无统计学意义。结论:颈部呈光滑杯形的ITI种植体非负荷期骨吸收程度大于圆柱形Anthogyr种植体,临床应考虑杯形颈部ITI种植体采用埋入式术式的可行性。Objective： To provide reference information for clinical implanting by comparing the differences of total bone absorption around two kinds of implants with cupuliform or column aspect cervical part during unloaded period.Method： Fifty-one implants（ITI group and Anthogyr group） with smooth cervical part were implanted in 45 patients with defect of dentition.Standardized dental radiographs were taken ten days and each month after implant placement.The radiographs were measured and analyzed by Photoshop8.0 software.Results： In ITI group,total crestal bone loss was1.3504±0.3212mm ten days after implant placement.The first month was 1.7874±0.3549mm and the 2nd,3rd month were1.8981±0.3662mm,1.9515±0.3858mm respectively.In Anthogyr group,total crestal bone loss was 0.6271±0.4015mm ten days after implant placement.The first month was 0.9025±0.4962mm and the 2nd,3rd month were 0.9779±0.5012mm,1.0250±0.5111mmrespectively.（P = 0.0020.05） Bone loss of the 3rd month among other period was statistically significant（P = 0.000 0.05）.The bone loss of ITI group was higher than Anthogyr group in ten days after implant placement.But there was no significant among the 1st,2nd,and 3rd month.Conclusion： Most bone loss was happened in first month especially in first the days after implants placement,which maybe in relation to regional accelerated phenomenon irritated by surgical trauma.Bone loss degree of ITI group was higher than Anthogyr group.Therefore,the availability of submerged ITI implant with cupuliform cervical part should be considered.
