颅内蛛网膜囊肿38例

摘　　要：探讨颅内蛛网膜囊肿（LAC）的临床诊断、手术适应证及手术方法。分析了38例病人的临床资料和影家学检查结果，对32例进行了手术治疗，其中囊肿全切3例，部分切除加周围脑池造口21例，单纯囊肿腹腔分流4例，部分切除加腹腔分流2例，颞肌填塞囊腔1例。结果手术病人中对倒随访1月~3月囊腔均有缩小，其中28例症状减轻或缓解，2例无变化，1例并发硬膜下血肿；24例随访6月~12月，19例囊腔明显缩小或消失，1例无变化，1例并发脑积水，3例癫痫症状缓解。LAC临床症状无特征性，诊断主要依据影家学检查结果，手术治疗应掌握好适应征，手术成功的关键是恢复正常的脑脊液循环。To study the clinical diagnosis, operation indicaions and tecniques of intracranial arachnoid cyst (IAC). The clinical data and image check results of 38 cases were analysised. Among 32 cases Who received operations, the cyst of 3 cases were removed completely,21 cases received partial resection and fistulation with periperial intracisterna. In 4 cases, the cysts were shunt on to abdominal cavity, 2 cases received both partial resection of the cyst and shunting operaion,temporal muscle was stufffed into cyst in one case. A follow-up study of 1 to 3 months was carried out on the 32 patients who received operations. In 31 cases, the cysts have ben contracted. The symptoms in 28 cases have reduced or relived. There was no improvement in 2 cases. And subdural hematoma occured in 1 case. A study was carried out on 24 cases followed up form 6 to 12 months. In 19 cases, the cysts became significently smaller or completely disappeared,1 case remained the same as before. Hydrocepholus apperaed in 1 case. Epilepsy was Under control in 3 cases. The clinical symptom of IAC shows no specificity. Thus, the diagnosis can drinly base on image check results. It is important to have a profound grasp of the indication. The key of the operation is to join fluid in the cyst to the normal ciroulation of cerebral - spinal fluid.
