344例ⅠⅡ期乳癌的远期疗效观察

摘　　要：作者随访334例临床ⅠⅡ期乳癌,结果提示:临床ⅠⅡ期乳癌无论采用传统根治或改良根治术,其5、10年生存率无差别;但单纯改良根治术后5年复发率高于传统根治术;放疗加化疗确能提高手术后(特别是改良根治术后)5年生存率,降低5年内复发率;手术后早期放疗和/或化疗不能改善10年生存率,降低10年复发率。因此,作者认为术后无条件行化疗和放疗的乳癌患者,尽可能行传统乳癌根治术。并强调手术后特别是改良根治术后放疗加化疗的必要性。生存5年以上的患者应定期化疗,局部复发或有远处转移者应结合手术行再放疗以提高10年生存率,降低10年复发率。344 coses of patients with stage I . II breast cancer are investigated by author and covjorkers. Our results indicate that the five - year and ten - year survival rates (65% , 32. 5% ) of stage I . II breastcancer patients treated with radical mastectomy(RM) have no significantdifference in comparison with those( 58. 33% , 23. 81% ) of the patientstreated with modified radical mastectomy (MRM); the five - year recurrentrate(62. 5% ) of the patients treated with MRM is higher than that( 35% ) ofthe patients treated -with RM; combination of post ~ operativechemotherapy and radiotherapy can in crease the five - year survival rate, and decrease the five - year recurrent rate, of the patients treatedivith MRM of RM; simple chemotherapy or radiotherapy can also leaver theffive year recurrent rate of the patients treated with MRM; however, early post - operative chemotherapy and I or radiotherapy fail to improvethe ten - year survival rale, and decrease the ten - year recurrent rate, of the patients treated with MRM of RM. Therefor, author emphases theimportance of post ~ operative chemotherapy and I or radiotherapy to thepatients with stage I . II breast cancer ', if post - operative chemotherapyand / or radiotherapy cannot be administered, radical mastectomy( RM) isproposed', moreover, periodic chemotherapy and I oar radiotherapy afterearly treatment ma improve the ten - year survival rate, and lower theten - year recurren rate, of the patients with stage I . II breast cancer.
