胃癌患者血清性激素水平及组织性激素受体的测定

摘　　要：目的:探讨性激素及其受体与胃癌的关系。方法:应用双抗体放射免疫分析法(DARIA)测定了80例胃癌患者和76例健康人血清睾酮、雌二醇及孕酮水平,同时用酶联雌二醇(E_2-HRP)、酶联孕酮(Pg-HRP)亲和组化法在80例胃癌标本石蜡切片上行雌激素及孕酮受体检测,分析了ST、SE、SP与ER、PgR的关系。结果:胃癌患者ST、SE、SP明显低于正常人(P<0.05),特别在ER阳性,PgR阳性,ER和PgR阳性病人下降更为明显(P<0.05),男性病人以ST下降为主(P<0.05),女性病人以SE下降为主(P<0.05);ER阳性率33.8％,PgR阳性率16.3％,女性病人有较高的ER阳性率,尤以≤55岁组为著(P>0.05),不论男、女,ER阳性大体分型以Borrmann Ⅲ、Ⅳ型多见(P<0.05),组织学上多见于低分化(粘液)腺癌,印戒细胞癌(P<0.05),呈弥漫性生长的胃癌ER阳性率高于非弥漫性生长的胃癌(P<0.05),ER阳性率还与局部淋巴结转移有关(P<0.05)。结论:ER和PgR存在于部分胃癌病人。胃癌可能是激素依赖性,它可能受激素因子的影响。AIM The correlation between serum testosterone estradiol progesterone and estrogen receptor progesterone receptor was analyzed METHODS The serum testosterone(ST)serum estradiol(SE)serum progesterone(SP)of 80 patients with gastric carcinoma and 76 healthy subjects were determined by method of double antibody radioimmunoassay,meanwhile the estrogen receptor(ER)and progesterone receptor(PgR)on paraffin embedded sections of 80 gastric carcinoma specimens were detected with E2-HRP and Pg-HRP histochemical methods.RESULTS The ST SE and-SP of gastric carcinoma patients,especially those with ER,PgR,or both positive,were significantly lower than that of the healthy subjects(P<0.05).The male patients showed a lower ST(P<0.05)and the female showed a lower SE(P<0.05);A total of 33.8% with ER positive and 16.3% with PgR positive were found.The female showed a higher ER positive rate,especially in those below 55 years old(P< 0.05).Both of the ER positive male and female were macroscopically common in Borrmann Ⅲ and Ⅳ(P<0.05).Microscopically,they were common in poorly differentiated(mucinous)denocarcinoma and signet ring cell carcinoma(P < 0.05).ER positive rate was higher in gastric carcinoma with a diffusely infiltrative growth pattern than that in non-diffusely infiltrative pattern(P < 0.05).ER positive rate was also associated with metastasis of the regional lymph nodes(P<0.05).CONCLUSIONS ER and PgR exist in some of the gastric carcinoma patients;Gastric carcinoma may be hormone dependent It may be influenced by hormone factors.
