口服／静注葡萄糖及皮下注射胰岛素后肿瘤内pH的变化

摘　　要：快速降低肿瘤内ｐＨ可增加肿瘤细胞对热疗的敏感性。测试了５８例表浅肿瘤内ｐＨ值，其中５０例（８６％）的基础ｐＨ值低于正常组织，中位７．２。肿瘤内各点处的ｐＨ也不尽相同，有１１例肿瘤测２～３个点的ｐＨ值，其值相差在０．０３～０．６０，中位数０．１８。２５例患者口服葡萄糖１００ｇ，有６０％的肿瘤内ｐＨ值下降０．１６±０．０３。１８例口服葡萄糖２００克可使肿瘤内ｐＨ值下降比例从６０％增至８３％，但下降幅度变化不大。静注葡萄糖５０ｇ可达到口服葡萄糖２００ｇ的效果；静注葡萄糖５０ｇ合并口服葡萄糖１００ｇ不能使更多的肿瘤内ｐＨ降低，但可使ｐＨ下降的时间从１２０分钟提前到６０分钟。口服葡萄糖合并皮下注射４０单位胰岛素不能改变肿瘤内ｐＨ下降比例和下降幅度。试验结果证实口服／静注葡萄糖可使肿瘤内ｐＨ快速暂时下降。AbstractIt is believed that rapid lowering of PH within tumor could sensitize tumor to hyperthermia.The intra-tumoral PH values of 58 superficial tumors were measured. It was found in 50 cases(86%), the basie PH value was lower than that of the normal tissue with a median value of 7.2。PH value also varied at different points within tl1e tumor. PH value varied from 0. 03 to 0. 60(me-dian value 0. 18) at 2-3 points in 11 tumors; 60%patients(15/25) who took 100g glucose orallywas found to have lowered PH value of 0. 16±0. 03. This variation of PH reduction was increasedto 83%in 18 patients who received 200g glucose orally, Intravenous 50g glucose combined withoral 100g glucose enhnced the PH reduction and made earlier the lowering from 120 minutes to60 minutes. Oral glucose Plus hypodermic injection of 40 units of insulin was unable to alter thevariation of intra-tumoral PH reduction. Nor was it able to alter the variation of intra-tumoralPH reduction and the extent of PH changes The authors believe that oral/intra-venous glucose isable to effect a rapid but transient lowering of intra-tumoral PH.
