造血干细胞在恶性淋巴瘤病人中的应用

摘　　要：对48例中晚期恶性淋巴瘤病人(HD14例,NHL34例,进行大面积、高剂量、化疗结合造血干细胞回输。具体方法及结果为:^(60)CO治疗组:一次TLI 6Gy,21例;8Gy,19例,其中10例采用自身骨髓回输。6MeV直线加速器治疗组:一次TLI 6Gy,2例;8Gy,2例。皆输注液氮保存的胎肝细胞。44例恶性淋巴瘤的1,3,5年生存率分别72％(30/44),57％(15/26)和30％(4/13)。HD14例分别为78％(11/14),75％(6/8)和50％(3/6)。大剂量化疗组:4例病人采用CTX3.0+VCR4mg/2天,同时回输自身外周血干细胞。结果CR2例,PR2例。三组病人皆于白细胞回升后在原发肿块处追加照射剂量30Gy/3周。临床使用造血干细胞结果表明在大面积高剂量放、化疗情况下,可以加速骨髓重建并提前恢复。Forty-eight patients with moderate and advanced malignant lymphoma were treated by large area and high dose radio-chemotherapy combined with hemopoietic stem cell reinfusion. 1.60Co total lymphoid irradiation (TLI) group; 21 cases received 6 Gy and 19 received 8 Gy. Ten also received autologous bone marrow infusion (ABMI). 2. 6 -MeV linear accelerator TLI group: 2 cases received 6 Gy and 2 cases received 8 Gy. They received fetal hepatic cells preserved in liquid nitrogen. The 1-5 3-, and 5-year survival rates of 44 malignant lymphoma cases were 30/44(72%), 15/26(57%) and 4/13(30%), respectively, while those with Hodgkin's disease were 11/14 (78%), 6/8(75%)and 3/6(50%), respectively. 3. High dose chemotherapy group: Four cases treated with'CTX 3. 0+VCR 4mg/2 d. They received autologous peripheral blood stem cell infusion as well. Two cases gave CR, the other two PR. Three groups of patients were boosted with 30Gy/3 -wk at the primary site after WBC recovered. Clinical use of hemopoietic stem cells could hasten the hemopoietic recovery relatively quickly after large area high dose radio-chemotherapy.
