苯妥英钠个体化给药方法:在癫痫病人中的应用

摘　　要：对11例癫痫病人的苯妥英钠(DPH)之药物代谢动力学参数进行了研究。Km均值8.12mg/L,波动范围0.66～28.04mg/L,个体之间差异42.5倍;Vm均值488.6mg/d,变动范围345.6～667.4mg/d,个体之间差异1.9倍,提示DPH代谢个体差异主要是由于Km的个体差异所致。同时,作者等还参照有关文献,介绍了几种实用的DPH个体化给药方案。Pharmacokinetic parameters of phenytoin were determined in 11 epileptic Patients. Their K_m values were 0.66-28.04mg/L, showing very great differences among different individuals; their V_m values were 345.6—667.4mg/d, showing relatively little disparaties, these results that the individual metabolic Kinetics of phenytoin ought to be mainly ascribable to the K_m variation. By utilizing the Michaelis-Menton formulae for K_m/L, V_m/d, Dosage/d (D), & Cone, stable serum/L (C_(ss)), and reviewing of related literature, the authors introduce several practical methods for ideal individlization ef the phenytoin C.
