摘  要:目的评价全身骨显像联合CT扫描对骨良恶性病变鉴别诊断的意义。方法选取有恶性肿瘤病史且全身骨显像有异常放射性分布但不能做出明确诊断的454例患者,于骨显像后行骨病变部位同期CT扫描。对两种影像技术进行综合分析,探讨全身骨显像联合CT扫描对骨良恶性病变鉴别诊断的意义。结果 454例受检者中,经两种影像技术检查后诊断骨转移271例,占59.69%(271/454);良性病变163例,占35.90%(163/454)。本组全身骨显像联合CT诊断骨转移瘤的敏感度为98.92%,特异度为97.27%,准确度为98.22%。全身骨显像联合CT扫描使骨病变的诊断率从单纯全身骨显像的71.19%(1 122/1 576)提高到了98.73%(1 556/1 576)。结论全身骨显像与CT技术融合具有重要临床应用价值,是骨良恶性病变诊断与鉴别诊断的重要技术方法。Objective To investigate the values of combined bone scan and CT in the differential diagnosis of benign and malignant bone lesions.Methods CT scans were performed on the lesions found but not diagnosed definitely by bone imaging in 454 patients with malignant tumor history.The differential diagnosis values of bone imaging combined with CT were investigated by analyzing the two imaging techniques comprehensively in the diagnosis of bone lesions.Results A total of 271 cases were diagnosed as bone metastases and 163 were finally diagnosed as bone degeneration after the two imaging examinations,accounting for 59.69%(271/454) and 35.90%(163/454) respectively.The sensitivity,specificity,and accuracy of bone scan combined with CT were 98.92%,97.27%,and 98.22%,respectively.The diagnosis rate of bone lesions increased from 71.19%(1 122/1 576) by bone scan to 98.73%(1 556/1 576) by bone scan combined with CT.Conclusion The combination of whole body bone scan and CT technology has significant clinical values and it is an important technical method in the diagnosis and differential diagnosis of benign and malignant bone lesions.
