儿童脊柱侧凸畸形的手术矫正

摘　　要：报告27例儿童脊柱侧凸,年龄2～16岁,侧凸角度40～118度。根据不同情况选择了6种术式:(1)“U”形棒法;(2)皮下哈氏棒撑开法;(3)单纯哈氏棒撑开加钢丝;(4)双哈氏棒撑开加钢丝;(5)哈氏棒-鲁氏棒联合手术;(6)分期手术。平均矫正率57.6%,平均输血600ml。并发症:脊髓神经一时性功能障碍3例;硬脊膜破裂、心衰肺水肿1例;切口皮缘部分坏死1例。文中详述和讨论了术前准备、术后处理以及预防和处理各种并发症的措施。Twenty-seven cases of child's scoliosis malformations, ranging in age from 2 to 16, are reported. Their Cobb's angles were from 40°to 1l8°. According to the different conditions, 6 types of operations were performed,namely U-shaped rob,single Harrington distraction rob with steel wirings, Harrington distraction rob under the skin,double Harrington distraction robs with steel wirings, combined Harrington distraction rob with Luque rob and stepwise operations. The average correction rate was 57.6% and average volume of blood transfusion was 600ml. There are some complications,such as 3 cases of spinal nervous dysfunction in short time, 1 case of rupture of dura mater of spinal cord accompanied by heart failure and 1 case of necrosis of partial skin edge of incision. The preoperative proparation, postoperative care and the various measures of preventing and dealing with some complications are discussed.
