带蒂大网膜充填术治疗慢性脓胸、支气管胸膜瘘(附17例病例分析)

摘　　要：本文报道我科1985年～1988年用带蒂大网膜充填术治疗慢性脓胸17例,其中9例合并支气管胸膜瘘,手术均获满意效果。术前、术后要充分引流,术中要彻底清除坏死物、肉芽组织及钙化灶,闭锁痿口。若网膜充填容积小于脓胸1/2时,应附加胸廓成形术。该手术创伤小、操作简单、适应症广。Seventeen cases of chronic empyema in our unit were treated with omental pedicle plombage from 1985 to 1988. Among them nine cases were complicated with bronchial fistula,thirteen cases were tuberculous empyema, and four cases were postoperative empyema.The result showed that sixteen cases were cured and one case improved. It suggests that empyema should be drained sufficiently before and after the operation, and bilateral or unilateral main bronchial intubation anaesthesia should be adpoted. This series of cases demonstrated that omental pedicle plombage is a safe, simple and effective operation in the treatment of chronic empyema complicated with or without bronchial fistula.
